      Job summary

      An opportunity has arisen in the Patient Flow and Discharge Team to recruit a Discharge Matron to support the Discharge and Patient Flow teams.

      Effective patient flow and timely and safe discharge is crucial to patients and families and in supporting the Trust in maintaining quality and performance standards.

      The successful post holder will be dynamic and support discharge and flow initiatives and processes to ensure patients at BHNFT experience the best possible care in terms of placement and discharge. They will support the Nurse Manager for Patient Flow in managing the existing teams and help to educate and support the wider teams in terms of discharge.

      The successful candidate will be expected to support the implementation of the new patient flow system (TeleTracking) and continue with the initiative moving forward.

      Main duties of the job

      The candidate will maintain a high standard of work and lead and support discharges processes throughout the Trust.

      They will support the Discharge Team and CBUs in ensuring that patient discharge is safe, timely and effective and highlight areas that can be improved to support this.

      They will work closely with partners to ensure the highest possible standards of care in terms of discharge for the Barnsley population.

      They will lead and support the implementation of the new Patient Flow System (Teletracking).
