Job summary
This role is a fantastic opportunity for apprentices who are currently studying for NVQ Level 2/3, apprentices should note that the salary would be Agenda for Change Annex 21 trainee rate (75%max of band) until they have completed their apprenticeship.

RightCare is Barnsley's Single point of Access for Health and Social Care staff supporting patient flow across Hospital and Community, coordinating admissions and discharges from Intermediate Care beds and Services, arranging support on discharge from hospital for Barnsley Residents.
We have a vacancy for a part time member of staff to join our friendly, successful team. Hours worked are between 8am-8pm, seven days, including weekends and bank holidays. Staff typically work long shifts but other shift working options can be considered.
We are looking for an individual who is willing to develop their knowledge and skills in relation to acute and community services, the successful person will need to work proactively as part of the wider team to deliver the RightCare Service for the benefit of patients and public.
For this role it is essential that you are motivated, enthusiastic, confident and organised. It would suit someone with previous experience in the acute and/or community care setting. You should have excellent interpersonal, communication, decision making and influencing skills in order to ensure the highest standards of care are achieved.
Main duties of the job
As a Patient Flow coordinator, you will be based in the RightCare department and will deal with referrals taken electronically and over the telephone. You will be part of a wider team including Nurses, administrative officers, Social Care and Intermediate Care staff. You will send referrals to a range of health and social care providers, including Care Homes, you will follow up the referral to confirm service provision and let the relevant teams know.
The aim of the service is to ensure that the right care is provided, at the right time and in the right place. You will work closely with health, social care teams and professionals from primary, secondary and community care supporting the RightCare nursing and coordination staff access care or other more suitable alternatives to hospital admission that will meet the needs of the service user.
This role aims to provide comprehensive and pro-active coordination and support, ensuring that all processes and tasks are undertaken efficiently, effectively and to a high standard, contributing to the smooth running and effectiveness of the service.
