      Are you an Advanced Clinical Practitioner looking for a new opportunity in a small, friendly practice?

      We have a new role to recruit 20 hours a week (Wednesday to Friday) and we would love to hear from you.

      Main duties of the job

      The role will be split between working with our care home in Barkston Ash one morning a week and forming part of our acute care provision at the practice for the remaining sessions.

      You will need to work collaboratively with the team to meet the needs of the patients whilst demonstrating safe clinical decision making and offering excellent care.

      About us

      Tadcaster Medical Centre is a busy, friendly six GP partner practice in a semi rural setting. We currently have one ANP, three Practice Nurses and an HCA working alongside the wider practice reception, dispensing and admin teams. We also train both GP Registrars and Medical Students.

      We have a list size of 9400 and work within the Tadcaster and Selby Rural PCN.

      Job description

      Job responsibilities

      Tadcaster Medical Centre wishes to successfully employ an Advance Clinical Practitioner to engage and support in the management of our Care Home patient population and support in the management of acute care for our practice population.

      This role is based on 5 sessions per week (2.5 days) to be spent either in the practice or in our allocated Care Home. The days of work will be Wednesday to Friday. At the care home the role will involve completing the weekly ward round, updating care and action plans as well as supporting acute medical issues on the day. In practice you will be seeing patients who have been triaged for acute care by the GP team or our ANP. You will be working and prescribing autonomously. Clinical supervision will be provided by the GP partners at the practice.

      The care home element of the role aims to reduce ad-hoc demand on services (outside of the weekly ward round) through advanced planning and coordination of care across multiple disciplinary teams. The role also provides greater capacity for General Practice whilst ensuring Care Home patients receive an excellent level of care. The role will act as the main clinical lead and resource between General Practice and the Care Home. This will be one half day each week.

      The remaining two days will be spent working within the clinical team at the practice.

      You may be asked to help support other workstreams to fit the needs of the business.
