      Job summary

      We are looking to recruit an enthusiastic Care Co-ordinator who will work as part of our friendly team. Ellergreen Medical Centre is part of the North Liverpool Primary Care Group. This role will be based in Ellergreen Medical Centre to assist in achieving PCN priorities.

      Main duties of the job

      To provide support to the Information Manager in ensuring maximum achievement in QOF; Local Quality Improvement Scheme (LQIS) and GMS Contract.

      To oversee the diary system for bloods for all patients within the practice.

      Play an integral role in the Online Triage Administration team.

      Assist in managing daily emails to Ellergreen.nhs.net

      About us

      Ellergreen Medical Centre is based in the Norris Green area of Liverpool. We have 10 GPs, 1 ANP, 4 Practice Nurses, 2 HCAs a Pharmacist and Pharmacy Technician. We also have a supportive and friendly administration team.

      Job responsibilities

      Job Plan Summary:

      To provide support to the Information Manager in ensuring maximum achievement in QOF; Local Quality Improvement Scheme (LQIS) and GMS Contract.

      To oversee the diary system for bloods for all patients within the practice.

      Play an integral role in the Online Triage Administration team.

      Assist in managing daily emails to Ellergreen.nhs.net

      Main Duties/Key Tasks:

      Manage maximum achievement for the QOF Depression register.

      Support maximum achievement for the QOF Non-Diabetic Hyperglycaemia register (IGR).

      Support the Information Manager with any further QOF registers to maximise achievement.

      Run the Mental Health call & recall system, liaising with community MH services.

      Run and action a number of regular searches as per our call & recall searches calendar. This includes, but is not limited to:-

      o Childhood Immunisations

      o Pregnancy pertussis vaccination

      o Named GP

      o Newly registered children

      o CKD New Diagnosis

      o Pneumococcal vaccination

      o Carer Annual Review

      o Emergency Prescription

      o FIT testing

      o Weight management

      Oversee the correct scoring and coding of Frailty patients and conduct annual falls risk assessments.

      Manage all requests for patient online access and to manage proxy access for patients within the practice.

      Support the Information Manager in the full implementation, promotion and running of any specific health / vaccination campaigns, e.g. breast screening; covid vaccination.

      Run search for Newly diagnosed cancer and Red flag symptom monthly lists and liaise with clinical cancer lead.

      Maintain and manage the waiting list of Cryotherapy appointments.

      Run and action the Rheumatoid Arthritis Call & Recall

      Manage and run the Shingles vaccinations call & recall.

      Generate blood forms and invite patients for their NHS Health Check.

      Action weekly Severe Combined Immune Deficiency (SCID) reports.

      Manage the Envisage screens and Auto Arrivals system.

      Play a key role within the online triage administration team to ensure online requests are appropriately processed and responded to.
