Job summary
An exciting and rewarding opportunity has arisen for an Administrator to join the team in the West Integrated Transfer of Care Hub (iToCH). Currently based in Camborne Health Office, Camborne. There would be an expectation for you to hot desk and shadow staff during your induction at Helston Community Hospital and Penwith Health Centre. You will play an important role in supporting clinical staff with admission avoidance, arranging community support, case managing non-urgent referrals and processing referrals.
You will help provide a seamless service for the people we serve, which will include cross boundary and collaborative working with the multi-disciplinary team, along with colleagues in Adult Social Care and other outside agencies in the community setting.
This is an ideal opportunity for an enthusiastic and dynamic individual. You will need to have good interpersonal skills, IT skills and possess excellent communication and organisational skills.
We will offer a supportive working environment in which to continue to develop your skills and knowledge in patient flow through a structured in-service training programme, appraisal and supervision systems.
This is a permanent, full-time post, working 37.5 hours per week to include weekends and bank holidays on a rota basis, as the service is available 8-8. The likely shift pattern would be 8-4 or 10-6 five days a week to include 1-7 weekends.
Main duties of the job
Monitor and review STRATA referrals to identify and process work to community health and social care teams:
o Managing a varied workload in a busy working environment. There is a requirement to be organised, prioritise workload and manage enquiries.
o Managing risk and priority of routine referrals, responsible for making the decision of the referral pathway.
o Review key blockages to patient flow, ensuring that patients achieve their most independent outcomes highlighting key issues to Clinical Prioritisers and to team managers.
o Monitor and assist in maintaining mailbox, phones and Electronic Referral system.
o Using clinical and professional systems to gather information as required to process referrals.
o Timely and accurate escalation of non-routine pathway referrals.
o Accurate and timely documentation of decisions on STRATA, RiO and Mosaic in line with organisational policies.
o To maintain compliance with organisational and statutory responsibility for confidentiality.
o Ensure proper use of the organisational resources in the course of business, ensuring minimal waste and minimal cost.
