What to Expect (Job Responsibilities):
- Communicate consistently with Account Management and leadership teams regarding new business plans and updates
- Provide exceptional customer service to patients and field intake calls
- Process referrals accurately and ensure timely compilation of required paperwork
- Support Account Managers in the field and facilitate coordination of services
- Utilize EMR systems effectively for managing patient referrals

What is Required (Qualifications):
- Minimum of one year of experience in hospice admissions preferred
- Two years of experience in healthcare data; LPN preferred
- Previous experience in customer service
- Strong computer skills in data entry and Microsoft applications; must type 50 WPM
- High school diploma or equivalent

- Experience in home health care, durable medical equipment, or skilled nursing facility
- Ability to build rapport with clients and account management
