      An Excellent Opportunity

      Mountain Valley has an opening for an ADT (Admissions, Discharges, Transfers) registered nurse for our two hospice inpatient units. The ADT Nurse is responsible for coordinating the admissions, discharges, and transfers of patients within the hospice inpatient units. This role ensures seamless transitions and continuity of care, maintaining the highest standards of clinical excellence and compassionate service.

      Key Responsibilities:

      Admissions:

      Conduct assessments for incoming patients, ensuring all necessary documentation and medical records are complete.

      Collaborate with physicians, social workers, and other interdisciplinary team members to develop and initiate individualized care plans.

      Provide education and support to patients and their families regarding hospice services, expectations, and care plans.

      Discharges:

      Facilitate the discharge planning process, ensuring patients and families are prepared for the transition.

      Coordinate with home care providers, outpatient services, and other community resources to ensure continuity of care.

      Complete all discharge documentation and provide clear instructions for ongoing care and follow-up.

      Transfers:

      Manage patient transfers within the hospice unit or to other healthcare facilities as needed.

      Communicate effectively with receiving homecare team, ensuring a smooth handover of care.

      Update patient records and inform relevant team members of transfer details.

      General Duties:

      Maintain accurate and up-to-date patient records in compliance with hospice policies and regulatory requirements.

      Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.

      Provide emotional support and counseling to patients and families, fostering a compassionate and supportive environment.

      Schedule

      • Monday – Friday
      • 11a – 8p
      • Evening and weekend as needed

      Qualifications

      • Valid Registered Nurse License
      • Minimum of 3 years of nursing experience, with at least 1 year in hospice field preferred
      • Strong clinical assessment and critical thinking skills
