      Job summary

      The Waterloo Practice is located in the HD5 area of Huddersfield. We have five approachable and supportive GP partners who with an Advanced Clinical Practitioner, a Nursing Team of 2 Practice Nurses and 3 Healthcare Assistants, Practice Management Team, Reception Team, Admin Team and 2 Secretaries, deliver first class care for approximately 9,600 patients locally.

      We have an exciting opportunity for an Advanced Clinical / Nurse Practitioner to join our Practice on a twelve month fixed term contract. Salary is dependent upon experience and is negotiable.

      Main duties of the job

      The Advanced Nurse Practitioner works autonomously and is accountable for their own professional actions

      Manages a clinical caseload

      Takes part in the On Call Rota

      Assesses, diagnoses and treats patients

      Assesses, diagnoses and treats patients in surgery or at their home who require acute medical attention with appropriate referral including admissions to secondary care for those with acute medical need

      Interprets a range of diagnostic tests and routine clinical procedures

      Makes critical judgements of the highest order to satisfy the expectations and demands of the job, eg. first contact with patients presenting acutely and manages care in the presence and absence of protocols

      Works within all relevant practice policies and procedural guidelines control

      Makes fast track referrals

      Undertakes annual reviews of patients understanding, ability to self-manage and reviews medication

      Supports On Call GP with face to face consultations, video and telephone consultations and appropriate triage

      The Advanced Nurse Practitioner works autonomously and is accountable for their own professional actions.

      · Manages a clinical caseload

      · Assesses, diagnoses and treats patients attending Primary Health Care services with a range of acute, non-acute and chronic medical conditions

      · Assesses, diagnoses and treats patients in surgery or at their home who require acute medical attention with appropriate referral including admissions to secondary care for those with acute medical need

      · Interprets a range of diagnostic tests and routine clinical procedures

      · Makes critical judgements of the highest order to satisfy the expectations and demands of the job, eg. first contact with patients presenting acutely and manages care in the presence and absence of protocols

      · Makes decisions where precedents do not exist, where appropriate without recourse to others, eg. direct referral of patients to Hospital Consultants without prior reference to other medical colleagues

      · Advises and supports others where standard protocols do not apply

      · Works within professional guidelines and codes of conduct

      · Works within all relevant practice policies and procedural guidelines, eg. infection control, chaperoning, risk management

      · Develops and manages highly specialised programmes of care and care packages and provides highly specialised advice concerning care normally beyond the scope of normal nursing practice, eg. establishes differential diagnoses, management/treatment plans and makes referrals as appropriate.

      · Makes fast track referrals

      · Communicates highly sensitive condition related information to patients, relatives and carers

      · Contributes to Practice targets, both local and national, within the disease management agenda, eg. QOF, prescribing incentive scheme

      · Audits outcomes of care against standards and initiate changes as necessary

      · Undertakes annual reviews of patients understanding, ability to self-manage and reviews medication

      · Supports On Call GP with face to face consultations, video and telephone consultations and appropriate triage

      · Contributes and supports The Waterloo Practice as a teaching unit

      · Contributes to delivering Federation incentives and other ventures as agreed by The Waterloo Practice

      · Partakes in monthly Practice Protected Time as arranged by the CCG for own development and development of the Practice

      Working Conditions:-

      · Frequent exposure to body fluids, blood, wounds, fleas and lice

      · Occasional exposure to aggressive behaviour

      · Frequent use of VDU equipment

      · Requirement to travel between different locations for attending patients in their own home, attending meetings, attending training etc.
