      An exciting opportunity has arisen within the Division of Intermediate Tier Services, rated OUTSTANDING by the CQC, we are seeking an advanced clinical Practitioners to join the Extensive Care service, predominately to work within our Acute Urgent crisis response service this is a blend of Community and Acute hospital work.

      Extensive Care is an Umbrella term as the service has a number of sub services within it.

      These are, an intermediate bed base within the Stamford Unit, this is a 96 bedded Unit over 3 floors. This is a blended approach to care with Therapy GP and an ACP workforce. We are also very proud to have are Urgent Crisis response team that work across our Neighbourhoods and support our Award-winning Digital Health Service, Virtual wards, along with having very strong ties to our intermediate urgent care team, this is a truly integrated team consisting of Nursing, Therapy and Adult social care. The Extensivist service is well established over 5 years, and sees patient in the community who have multiple hospital attendances, along with long term conditions and complex health needs. We are really excited to be developing our acute frailty service and Community frailty service

      Main duties of the job

      Urgent Crisis response is an Advanced Clinical Practitioner led service based in the community. The team provide urgent health care to patients in their homes who are experiencing an acute illness, to avoid an unnecessary admission to hospital.

      Working within the Urgent Crisis Response team you will be part of developing and shaping the future of the two - hour response service within Tameside and Glossop.

      The post holder will be an autonomous Advanced clinical practitioner who will be expected to take an active role in the response, stabilisation and making safe element of acute care pathways by delivery of advanced assessment, management, and treatment of patients over 18 years of age within the urgent crisis response service.

      The successful postholder will have community experience and demonstrate specialist clinical skills and knowledge in advanced clinical assessment. The Advanced clinical practitioner will offer clinical expertise, guidance, and management plans, clinical decision making, and discharge planning to acutely unwell patients. Develop differential diagnosis and work in collaboration with primary care, community services and acute trust and the wider multidisciplinary team to agree what services need to be put in place; what clinical and medical support will be needed to manage and mitigate risk to ensure a safe and supportive care in their usual place of residence.
