      Job summary

      Hours: 4 sessions- 2 sessions on a Wednesday and 2 sessions on a Friday- to be worked by one person or could be split between two people.

      As a skilled Advanced Clinical Practitioner with the Humber Virtual Frailty Ward, you will be delivering holistic care to frail and vulnerable patients across Scarborough, Whitby and Ryedale.

      Based at the teams Community Hub in Scarborough, you will work as member of a multidisciplinary team that includes Advanced Complex Care Managers, a Senior Physiotherapist, a Specialist Clinical Pharmacist and Nursing Associates. The team is supported daily by a Frailty General Practitioner working remotely in our innovative York Community Frailty Hub, and is able to draw on the expertise and resources of other Humber Community teams such as District Nurses and Heart Failure Specialist Nurses.

      Main duties of the job

      Duties and Responsibilities:

      The duties and responsibilities to be undertaken may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Medical Director or Board of Directors, dependent on current and evolving workload:

      Clinical responsibilities:

      • Assess the healthcare needs of complex patients with both defined and undifferentiated or undiagnosed problems, carrying out holistic face-to-face patient assessments and developing comprehensive management plans based on Comprehensive Geriatric Assessments (CGAs)
      • Manage patient risk effectively and safely, including high levels of complexity and uncertainty
      • Utilise other clinical staff appropriately to ensure safe and effective patient care, demonstrating awareness of your own limitations and the confidence to escalate when required
      • Contribute to the clinical management of patients as appropriate, including initiating investigations, reviewing results, liaising with secondary care, and making referrals to other providers and services as appropriate including initiation of effective emergency care
      • Contribute to Advance Care Planning and Treatment Escalation Plans through completion of CGAs & ReSPECT forms, including best interest discussions and decisions where required

      • Provide information and advice on prescribed and over-the-counter medication on medication regimens, side-effects and interactions
      • Work with and support other members of the MDT, including but not limited to Advanced Complex Care Managers, Nursing Associates, Specialist Pharmacists, Physiotherapists, Urgent Care Response Paramedics and other Frailty GPs.
      • Promote the application of standards and codes of Practice set by the Nursing and Midwifery Council or Health and Care Professions Council, Department of Health, British Geriatrics Society and local NHS trust
      • Participate in national frailty training and maintain a high volume of CPD in Frailty-related conditions, with the aim of obtaining the Diploma in Geriatric Medicine
