- Comprehensive Patient Assessment:Conduct thorough assessments, including physical examinations, patient history taking, and interpreting medical tests, to diagnose and manage a variety of conditions.
- Prescribing and Treatment Planning:Prescribe medications, request appropriate investigations (e.g., blood tests, X-rays), and develop comprehensive treatment plans in partnership with patients.
- Patient Education and Counselling:Provide patients with information and advice on their conditions, medications, and treatment options, empowering them to actively participate in their care.
- Collaboration and Communication:Work effectively within a multidisciplinary team, communicating with other healthcare professionals (e.g., GPs, nurses, specialists) to ensure seamless patient care.
- Managing Complex Cases:Demonstrate expertise in managing patients with long-term conditions, complex health needs, and acute illnesses.
- Mentorship and Support:Support and mentor other members of the nursing team, including Practice Nurses and Healthcare Assistants, contributing to their professional development.
- Quality Improvement:Participate in quality improvement initiatives, contributing to the development of new services and pathways to enhance patient care.
- Applicants should provide evidence that their qualifications, competencies and capabilities map to the HEE Framework (2017) via the Health Education England (HEE) Centre for Advancing Practice by completing one of the following:
- -an HEE accredited Advanced Practice Programme
- -an MSc in Advanced Practice mapped to the four pillars of advanced practice
- -the Supported e-Portfolio Route.
- The successful applicant should be a non-medical prescriber with extensive experience of Primary Care and/or General Practice. We are keen to meet applicants with qualifications and experience managing long-term conditions and/or family planning and/or research
- Applicants should provide evidence that their qualifications, competencies and capabilities map to the HEE Framework (2017) via the Health Education England (HEE) Centre for Advancing Practice by completing one of the following:
