      Main duties of the job

      Working autonomously within your professional scope, you'll provide care across the full patient pathway fromhistory taking and clinical assessmenttodiagnosis, treatment and follow-up. You'll handle acute, same-day care viaface-to-face and telephone consultations, and contribute to proactive care planning and liaison with external partners.

      As a training practice we provide a Clinical Supervising GP to support all our staff throughout the day.

We have a strong ethos on continuous learning and offer training opportunities and clinical support together with an enthusiastic multi team approach to care. (Urgent Care team, Specialist nurse led clinics, health coaches, nursing associates, F2 and Medical Students).

      You will work closely with our GPs to advise on treatment and healthcare for patients, to include those presenting in the primary care setting and those in the community.

      You will triage acute patients presenting in the surgery environment.

      You will attend home visits to include residential and nursing homes

      You will work collaboratively with the multi-disciplinary team to meet the needs of our patients.

      Job Description:

      • Assess patients presenting with acute illness.
      • Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
      • Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
      • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and follow up as appropriate.
      • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
      • Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan
      • To provide assessment, treatment and diagnosis at point of first contact by attending to patients in a variety of clinical or non-clinical settings according to patients needs
      • To undertake home visits in accordance with the relevant protocols
      • To make professionally autonomous decisions for which they are accountable
      • To refer patients to an alternative care setting or treat and discharge as appropriate
      • To instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports
      • To ensure that professional standards are maintained and within the guidance provided by the Department of Health, the Health and Care Professions Council (HCPC)
