Job summary
We are seeking an Advanced Clinical/Nurse Practitioner to join our friendly forward thinking team. The ideal candidate will have experience of working within Primary or Secondary Care in an ACP role and will be able to work autonomously seeing complex patients alongside the wider MDT.
Main duties of the job
To assist the medical personnel in the care of practice patients by providing assessment, screening treatment services and health education advice. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, minor illness. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Undertake care for patients in their homes as required by the Partners. Be competent and confident in managing people with long term health conditions. Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug- based treatment methods into a management plan. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
