Due to retirement, we are looking for a highly motivated ANP/ACP with experience in primary care. The candidate must enjoy a challenge and wide variety of work. You will be caring, patient and be adaptable to individual patient needs. You will be joining existing team of 3 ANPs and will be supported by well-established clinical and administrative teams.
Main duties of the job
The role involves telephone and online triage, face to face consultations as well as providing home visits (where needed). You must be comfortable working independently, with ongoing support from the wider team.

ü Part-time hours (18-22 hours) to be worked Wed-Friday. This includes morning and afternoon shifts.

About us
Old Station surgery is based in Ilkeston and we are a large practice split across 3 sites with a list size of approximately 15,500 patients. We are a training practice with Good CQC rating and use SystmOne.
Ideally, we would like you to start as soon as possible, but we are happy to wait for the right candidate.

To manage your own caseload and provide care for patients presenting with undifferentiated or undiagnosed conditions, including detailed history taking, clinical assessment, diagnosis, treatment and evaluation of their care, ensuring patient choice. To demonstrate safe, clinical decision-making and expert care for patients within general practice whilst working within the boundaries of your role. To develop and use referral pathways for ANPs to the local acute NHS Trust services and Diagnostic and Treatment Centres.
Working collaboratively with the practice staff and the wider multi-disciplinary team to meet the needs of patients and support the delivery of care in line with policy and procedure.
