      Job summary

      Lionwood Medical Practice is looking for an experienced Advanced Nurse Practitioner (ANP) who has worked in a GP surgery in the UK. The successful candidate will be responsible for diagnosing and treating patients presenting with minor illnesses, as well as advising them on their continuing medical and nursing needs. This role involves triaging patient needs and making necessary referrals for further investigation or treatment.

      The candidate will work according to practice protocols and guidelines, delivering care independently and in collaboration with GPs and the wider practice team.

      The candidate will also support the practice by being one of the triaging clinicians alongside other ANPs and GPs.

      Main duties of the job

      Main Duties and Responsibilities

      • Patient Consultations: Provide access to primary care for patients presenting with various conditions, both in surgery, utilising video consultations and making calls over the phone. These types of appointments will be a combination of pre-bookable and on the day appointments.
      • Caseload Management: Assess, diagnose, and manage patients autonomously, prescribing evidence-based care, including medications, and referring patients to consultants or other professionals as needed.
      • Care Planning: Develop ongoing health plans with patients and their families, emphasizing prevention and self-care, and following up on patient progress.
      • Health Promotion: Support patients in adopting healthy lifestyles and self-care strategies. Including taking part in promotion campaigns within the practice.
      • Mental Health Support: Assess, identify, and refer patients with mental health needs in accordance with the National Service Framework (NSF) for Mental Health. Working with the practice mental health team and external partners and organisations.
      • Collaboration: Work with the multidisciplinary team to ensure holistic and comprehensive patient care.
      • Record Keeping: Maintain accurate, up-to-date patient records on the practices computer system (SystmOne).

      Pathological Specimens and Investigations

      • Process and communicate results of recent tests to patients and ensure that a care plan is in place.

      • Ensure Practice safeguarding protocols are adhered to at all times, escalating incidences where appropriate.
