      Job summary

      An exciting opportunity has arisen for an experienced and enthusiastic Advanced Nurse Practitioner to join our friendly, dedicated and established team based in Chesham.

      As a member of our clinical team you will be working alongside our GPs and other allied healthcare professionals to provide the very best patient care.

      Main duties of the job

      The successful candidate will be required to work independently, within the scope of their professional boundaries, to assess and manage patients. You will be providing assessment, treatment, and diagnosis to patients via face-to-face consultations. Consultations will be held in surgery. You will be seeing patients with acute and emergency primary care clinical presentations.

      You will need to demonstrate safe, clinical decision-making and expert care including assessment and diagnostic skills. There will be a need to prioritise and triage the needs of patients making any necessary referrals for investigations as appropriate.

      You will need to be a qualified Advanced Nurse Practitioner, with an MSc or equivalent relevant nursing degree. An independent prescribing qualification is preferable, as well as prior experience of working in Primary Care.

      About us

      The New Surgery is a stable six-partner Practice supported by an excellent, dedicated team of nurses and admin /reception team.We also benefit from a wide multi-disciplinary team, including clinical support from a Pharmacist, Physiotherapist and Nursing Associate.

      We are a GMS practice, with a list size of 11,000, based in a purpose-built medical centre in Chesham with free parking available on site.We share the premises with another GP surgery and a dentist.There is also a pharmacy on the ground floor.

      We are an EMIS practice, rated GOOD by CQC and we consistently receive excellent results in the National Patient Survey.

      Our current ANP is leaving us to pursue a new opportunity abroad and we are keen to find a replacement who will fill this established and fully integrated role within our clinical team.

      Job description

      Job responsibilities

      The following are the main responsibilities of this role. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

      • Manage patients presenting with a range of acute medical conditions
      • Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
      • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
      • Undertake the collection of pathological specimens
      • Request pathology services as necessary
      • Process and interpret pathology and other test results as required
      • Maintain accurate clinical records in conjunction with extant legislation
      • Chaperone patients where necessary
      • Support the team in dealing with clinical emergencies
      • Recognise, assess and refer patients presenting with mental health needs
      • Be an extended and supplementary prescriber
      • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
      • Contribute to practice targets (QOF, etc), complying with local and regional guidance
      • Support the clinical team with all safeguarding matters in accordance with local and national policies
      • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
      • Deliver opportunistic health promotion where appropriate
      • In addition, the Advanced Nurse Practitioner may be requested to:
      • Undertake audits as required
      • Participate in local initiatives to enhance service delivery and patient care
      • Support and participate in shared learning within the organisation
      • Continually review clinical practices, responding to national policies and initiatives where appropriate
      • Participate in the review of significant and near-miss events
      • Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required
