- Diagnosing and treating patients presenting to primary care with acute medical conditions, minor illness and mental health problems, including a mix of face to face and telephone consultations.

Job summary
One Ellesmere Port Primary Care Network (PCN) is a collaboration of the 6 GP Practices from across the town. Working together at scale to develop better services for our patient population of approximately 72,000 patients.
Whitby Health Partnership is a core member Practice of the PCN and the second largest Practice.
The successful candidate will be employed by and based at Whitby Health Partnership focusing on their patient population. They will join a valued and existing team of 3 Advanced Nurse Practitioner's, 13 GP's and 2 nurses here at Whitby Health Partners.
Main duties of the job
The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC requirements for advanced practice must be met.
