- Being able to conduct a comprehensive physical health assessment of patients with complex multiple healthcare needs.
- Interpret results of multiple different assessments and investigations to make a diagnosis, and plan to deliver care.
- Prescribe and work with patients to manage their medicines, and to be able to make onward referrals where required.
- Competently make ethical, evidenced decisions and interventions when faced with complexity and assess the risk associated with these decisions
- Comfortable working both independently but also as part of a multi-disciplinary team.
- Comfortable with IT, our clinical systems include SystmOne and AskmyGP. (Full training and support of these systems can be provided).
- To work as Duty Buddy on selected days to help the Duty Doctor with the management of tasks, admin and results.

- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients
- Implement and evaluate individual specialised treatment plans for chronic disease patients

- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
- Deliver opportunistic health promotion where appropriate
- Participate in the administrative and professional responsibilities of the Practice Team
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patients care record
- Ensure accurate completion of all necessary documentation associated with patient healthcare and registration of the practice
- Ensure collection and maintenance of statistical information required for regular adhoc reports and audits
- Ensuring when significant events are identified, they are recorded appropriately on GP Team Net.

Job summary
St John's Medical Centre has an exciting opportunity for an Advanced Nurse Practitioner to join our enthusiastic and supportive Practice Team. The successful candidate must hold prescribing qualifications, be organised, motivated and caring to ensure the highest levels of care are delivered to our patient population through patient assessments, diagnosis and clinical management.
St John's Medical Centre is a large, fast paced GP surgery located in Grantham, offering excellent transport links with easy assess to the A1 and on the mainline for London. Two notable Grammar schools in Grantham are Kesteven and Grantham Girls School and The Kings School. We currently have circa 13,500 patients registered at the Surgery and retained our 'Good' rating in our latest CQC Regulatory Review.
We are flexible with regards to the number of hours for the role but would be looking for 30-37.5 hours per week, working Monday- Friday between the hours of 08:00am-18:30pm.
Main duties of the job
Working as an Independent Practitioner, the successful candidate will be required to assess, diagnose and manage patients with a variety of conditions. We offer a same day service and their maybe a requirement for occasional home visits to be undertaken.
