Job summary
Due to ongoing expansion of our practice list size, Ratby Medical Centre and looking to recruit an additional Advanced Nurse Practitioner, on a permanent basis, in order to support our growing patient population.
Job summary
We are looking to recruit an ANP for approximately 20 hours per week, ideally over 4 or 5 days per week, and have you join our friendly and productive team.
There will be a degree of flexibility with the working pattern to support the right person and we have an open door policy for all members of the clinical team to discuss complex medical challenges and queries.
We are a Systm1 practice and have an interest in training medical students within the practice.
As a practice, we do not support any care homes and have very few home visits due to the nature of our patient demographics. However, this may change as the practice and the local area continues to grow.
Main duties of the job
We are looking to increase our clinical capacity within the practice to support our increasing list size and would love to welcome the right person to join our close and happy team. We would be keen to support the right candidate to continue to develop their skills within General Practice and would encourage you to talk to us about your areas of interest.

Ratby Medical Centre is a small but hardworking family run practice, who are enthusiastic about caring for our patients.
We have recently moved into a brand new building, which we designed, and as a practice, we have grown significantly over the last six to seven years. We are looking to increase our clinical team to support quality care for our patients as part of our practice vision.

Job responsibilities
The Advanced Nurse Practitioner will work autonomously and is accountable for their own professional actions.
The role will include:
Provide the choice of direct access to a nurse practitioner, both in the practice and over the telephone, for the general practice population.
May see both self-referred and internally referred cases and make professional autonomous clinical and prescribing decisions in relation to their problems for which she/he is accountable.
Provide a first point of contact within the Practice for patients presenting with selected undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.
Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
Be able to prescribe safe, effective and appropriate medication as defined by current legislative framework as appropriate.
Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards.
Provide safe, evidence-based, cost-effective, individualised patient care.
Offer a holistic service to patients and their families, developing, where appropriate, an on-going plan of care/support with an emphasis on prevention and self-care.
Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.
Refer patients directly to other services/agencies as appropriate including seeking GP advice if and when appropriate.
Work with nursing, medical and health care assistant colleagues to ensure that latest NHS directives and long term chronic disease management are being delivered.
Work with multi-disciplinary teams within the Practice and across the wider locality to promote integrated and seamless pathways of care.
Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.
Help develop and set up new patient services and participate in initiatives to improve existing patient services.
Participate in audits and inspections as appropriate.
Care and Programme Management
Develops and manages highly specialised programmes of care and care packages and provides highly specialised advice concerning care normally beyond the scope of normal
Nursing practice (e.g. establishes differential diagnoses, management/treatment plans and makes referrals as appropriate). Makes fast track referrals.
Communicates highly sensitive condition related information to patients, relatives and carers.
Initiates the process of diagnosis with patients suspected to have a chronic disease (e.g. diabetes, COPD, asthma, IHD) referring to other clinical staff as appropriate.
Contribute to practice targets both local and national within the disease management agenda (e.g. QOF, prescribing incentive scheme).
Contributes to disease registers.
Audits outcomes of care against standards and initiate changes as necessary.
Undertakes annual review of patients understanding and ability to self manage and medication reviews.
Refers to other members of the extended primary health care team as appropriate (e.g. Chiropodist, Dietician, Specialist Nurses).
Actively implements safeguarding protocols for children and vulnerable adults and to know who to refer to and how to escalate concerns about radicalisation.

Work within the latest NMC Code of Professional Conduct.
Maintain their professional registration.
Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of revalidation, audit and peer review.
Participate in continuing professional development opportunities (attending necessary courses & study days) to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.
Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation.
Work collaboratively with colleagues within and external to the practice.
Pro-actively promote the role of the ANP within the Practice and externally to patients, key stakeholders and agencies.
