- Act as an advanced practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
- See pre-booked nursing appointments as if working in a conventional GP Surgery
- Undertake consultations, both face to face and via the telephone, with patients of all ages (neonate to elderly) as an autonomous practitioner and using own clinical judgement to diagnose, treat, refer and/or discharge patients
- Make direct referrals to primary, secondary and social services within our agreed pathways, guidance and protocols.
- Administer dressings
- Provide chronic disease management for pre-booked patients when allocated to specifically competent ANPs.
- Ensure that patients receive high quality clinical care, delivered in a timely manner.
- Prescribe, supply and administer medicines as indicated to address patient need.
- Ensure complete and accurate documentation
- Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patients needs are met.

Job summary
Due to organisational growth we currently have vacancies for Advanced Nurse Practitioners
The primary focus of the role is work as an autonomous advanced practitioner providing high quality and timely patient centered care.
The post holder will be responsible for the assessment, diagnosis, treatment, referral and discharge of a diverse range of patients with minor injuries/minor illnesses, without reference to a doctor and within their level of competence and agreed protocols.
This job role is aligned to that of a traditional in-hours Advanced Nurse Practitioner working in General Practice and has fundamental differences to that of the Out of Hours ANP Role.

Educational/Personal Development
Work with the Operations Manager and Clinical Director to:
Develop and maintain an excellent learning environment
Develop and implement a range of learning activities for the nursing team
Support the development of other members of the nursing team.

Participate in defining own developmental plan to maintain and develop own clinical skills and ensure own professional development.
Undertake and complete all statutory and mandatory training.
Participate in an appraisal and regular performance reviews.
