Job Description

- To carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
- To assess, triage, diagnose and treat patients face to face accordingly, performing specialist assessment of their needs and make referrals as necessary.
- To request diagnostic investigations and interpret within agreed protocols e.g x-ray, blood tests and scans.
- To clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
- As an independent prescriber, to review and prescribe medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols.
- To deliver high quality, compassionate care and treatment to patients with a focus on promoting health, self-care and independence using a personalised and holistic care approach.
- To contribute to local and national practice targets e.g QOF and Prescribing incentive scheme.
- To use Arden's templates to ensure accurate and legible notes of all consultations and treatments are recorded in the patient's records
- To deliver high quality, compassionate care and treatment to patients with a focus on promoting health, self-care and independence using a personalised and holistic care approach.
- To contribute to local and national practice targets e.g QOF and Prescribing incentive scheme.

- Minimum of 5 years post registration experience and recent experience in a primary care/community setting.

- Proven ability to evaluate the safety and effectiveness of their own clinical practice.
- Knowledge of diabetes including initiation of insulin to patients.
- 1. Degree in Nursing and UK NMC Pin Qualified
- 2. Master of Science in Advanced Nurse Practitioner
- RCN Minor injuries
- Certificate in Diabetes Care
- Managing patients with Chronic Diseases

About us
South Hornchurch Medical Practice has approximately 6000 patients and we are located in a health centre with parking on site. You will be working alongside 1 Lead GP and 3 female locum doctors, an Advanced Clinical Prescriber, a Practice Nurse and HCA with additional PCN Staff including a Pharmacist, Physiotherapist, Dietician and a Social Prescriber. We strive to deliver the highest quality healthcare to our patients through continuous training and development of staff.
