      Job summary

      This post is a Fixed Term/ Secondment until 31/03/2026. If you are a Hywel Dda University Health Board employee and are applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application.

      A new and exciting opportunity has arisen to be part of the Pharmacy team in PPH and the multidisciplinary inflammatory bowel disease (IBD) team in Carmarthenshire.

      This post is part time, 26.25 hours a week and temporary for one year to cover maternity leave (15 hours with the IBD team and 11.25 hours with pharmacy).

      The successful candidate will work alongside the consultants and specialist IBD nurses to optimise treatments for IBD patients and will run Pharmacist led clinics including virtual clinics. It is desirable for the post holder to be a non- medical prescriber.

      As an advanced Pharmacist you will be responsible for the co-ordination and delivery of a patient focused clinical pharmacy service. The successful candidate will work within general medicine developing their Pharmacist role e.g. working with the multidisciplinary team, monitoring financial expenditure, undertaking research and service improvement projects. The post holder will deliver educational sessions to medical and nursing staff at PPH, and support the development of the technician workforce.

      Main duties of the job

      The post holder will develop clinical skills in order to undertake patient assessment of disease activity and formulate clinical management plans.

      The post holder will review patients on immunomodulatory and biological drugs to ensure optimum treatment and appropriate monitoring including switching and adjusting treatments where appropriate.

      The post holder will monitor financial drug expenditure for the specialty and lead on the implementation of cost and service improvements programmes for the speciality.

      The post holder will work with the multidisciplinary team and with the health board's homecare team and homecare providers. They will identify new prescribing trends, new products and national guidelines for the speciality, and monitor their implementation.

      Professional development is of utmost importance and support and training will be provided. We are a patient-centred and progressive team who work closely with the multidisciplinary teams enhancing patient safety through professional advice to prescribers, discharge planning and attending consultant ward rounds.
