      ROLE OVERVIEW and PURPOSE

      The Advanced Practice Practitioner, Street Medicine will serve as an integral component of the Street Medicine program. This role will encounter and manage a shifting landscape to meet the healthcare needs of the homeless and underserved populations. Additionally, this role will provide an unparalleled opportunity to impact lives directly where it is most needed in the community and in the clinic.

      RESPONSIBILITIES

      • Serves as a primary care provider by offering compassionate and comprehensive medical services in non-traditional settings, such as community streets, encampments, and temporary housing
      • Adapts practice methods to meet the varying conditions and resources available in clinical settings and street-based environments
      • Works closely with a cross-functional street medicine team to innovate, expand, and refine the service delivery model
      • Spearheads educational initiatives for team members, fostering a culture of continuous learning and adaptability
      • Embraces the unpredictable nature of street medicine by using ingenuity and resourcefulness to overcome challenges
      • Engages in case management by demonstrating agility in responding to unexpected patient needs and situations
      • Provides urgent care for unsheltered homeless patients with acute/chronic medical and/or psychiatric conditions, including providing medications

      Direct Patient Care:

      • Performs comprehensive physical examinations on patients, including review of patient medical data, such as health history
      • Orders, administers, and interprets test results for diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests
      • Performs therapeutic procedures such as injections, immunizations, suturing, and wound care, and managing infection
      • Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance
      • Impacts patient experiences by demonstrating courteous and helpful behavior and a commitment to accuracy
      • Assesses and provides age-appropriate care to patients

      Team Support:

      • Shares accountability for overall patient health outcomes, working in coordination with care teams
      • Operates to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholders
      • Responds promptly and with caring actions to patients and employees
      • Maintains professional working relationships with all levels of staff, clients, and the public
      • Cooperates in accomplishing communicated department goals and objectives

      Clinical Administration:

      • Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care
      • Performs in accordance with NHC practice protocols and under the guidance and supervision of the supervising MD(s), according to the terms of the Supervisory Agreement

      EDUCATION/EXPERIENCE

      • Master’s degree from an accredited physician assistant or nurse practitioner program
      • Valid CA Physician Assistant or Nurse Practitioner license
      • Valid CA DEA license
      • Valid BLS certification from approved American Health Association provider required upon hire
      • Experience prescribing buprenorphine for medication assisted treatment preferred
      • Experience with psychiatric medication management preferred
      • Bilingual (English/Spanish) preferred

      ADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)

      • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
      • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
      • Knowledgeable about and experience with word processing programs, spreadsheets, or electronic medical records systems
      • Ability to serve marginalized communities
      • Ability to improvise and problem solve in non-standard environments
      • Ability to work through ambiguity and identify best practices and devise solutions
      • Ability to successfully manage multiple tasks simultaneously
      • Excellent planning and organizational ability
      • Ability to work as part of a team as well as independently
      • Ability to work with highly confidential information in a professional and ethical manner

      Physical Requirements

      • Ability to lift/carry 20 lbs/weight
      • Ability to stand for long periods of time

      Pay range:

      • $162.1K - $219.3K, per year depending on experience. (DOE)
