      Job Description:

      Acclaim Physician Group has an opportunity for an Advanced Practice Provider to join the Primary Care team at the JPS Stop Six Clinc. Located in Fort Worth, TX, this large clinic provides primary care, women’s health, dental, laboratory, radiology and a pharmacy under one roof. This clinic is comprised of a fantastic team of Family Medicine, Internal Medicine, Pediatric, and Advanced Practice Professionals.


      As the county hospital system, JPS has been serving the residents of Tarrant County for over 100 years. This is a great opportunity to treat complex medical issues, and to provide compassionate, quality care to those patients that need it most.

      Job Summary:

      The Advanced Practice Provider in this role will be working with all age goups the supervision of the supervising physician and is responsible for assisting with all aspects of patient care, including diagnosis, treatments and consultations within the inpatient and/or outpatient setting. This position will also educate patients about preventative care and prescribed treatments; may also conduct physicals, order tests and serve as a patient's healthcare provider.

      Duties and Essential Job Functions:

      • Performs assessments of patient’s age 0-22 years to assess for general health, to identify disease, diagnoses and treats medical conditions documenting the care plan.
      • Provides comprehensive primary care, manage acute and chronic illnesses and link these services with other community resources
      • Provides patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verify and document that the patient understands diagnosis, treatment and follow up recommendations.
      • Develops patient treatment plans in conjunction with the patient, parents or guardian.
      • Attends Performance Improvement Program and Peer Review committee meetings as needed and be an active participant in the process.
      • Performs other job related duties as assigned.




      Qualifications:

      Required Education and Experience:

      · Graduate of an accredited Physician Assistant program OR Nurse Practitioner program (FNP or PNP certification)

      Preferred Education and Experience:

      · Experience in a community health setting or public health clinic setting.

      · Experience with practice management systems and EMR – Epic preferred

      · Experience with Adolescent population

      Required Licensure/Certification/Specialized Training:

      · Current Texas Physician Assistant License OR by the Board of Nurse Examiners for the State of Texas or proof of reciprocity of licensure between the State of Texas and another state

      · Basic Life Support (BLS) certification

      · Advanced Cardiac Life Support (ACLS) certification
