- Must be licensed or license eligible in the state of Ohio
- Physician Assistant must have a Master’s degree - or - Nurse Practitioner must have a Master of Nursing (MSN) or Doctor of Nursing (DNP) degree
- Certificate of Authority to practice.
- Current American Heart Association (AHA) CPR certification.
