      Position Summary
      • Responsible for family medicine/primary care within the medical group under the supervision of an attending physician.

      Education
      • Master’s or Bachelor’s degree in Physician Assistant Studies from a CAHEA accredited program
      • Master’s Degree in Advanced Practice Nursing Studies from a CCNEA accredited program.

      Licenses and Certification
      • Current Illinois Licensed Physician Assistant or Advanced Practice Nurse
      • Current NCCPA certificate as a Physician Assistant or AACC certificate as an Advanced Practice Nurse
      • Active DEA license or ability to apply for such license prior to or upon hire

      Role Specific Responsibilities

      • Assesses, diagnoses, and determines/alters treatment and management plans appropriate for age, acuity and clinical condition. This includes ordering, performing and interpreting appropriate diagnostic studies and prescription of pharmacologic and non-pharmacologic interventions and therapies in accordance with approved protocols/policies.

      • Manages conditions based on clinical indication, evidence-based care, cost effectiveness and assessment of risks/benefits and alternatives.

      • Provides care that is patient and family centered, compassionate, appropriate and effective for the promotion of health, prevention of illness and treatment of disease throughout the lifespan.

      • Facilitates consistent, coordinated care and clear communication among all members of the healthcare team.

      • Manages patients as part of an interdisciplinary team and within scope of practice. Formulates and implements outcomes-based treatment plans based on assessments, evidence-based medicine, standard professional practice guidelines and in collaboration with physicians and other interdisciplinary team members when appropriate.

      • Performs procedures specific to scope of practice and knowledge of the provider as delegated by supervising Physician.

      • Maintains accurate, complete, concise and timely documentation in the EHR. Substantiates and submits professional services consistent with compliant coding and billing practices.

      • Act as a liaison between healthcare providers, the patient, family, and payers.

      • Continuously build and maintain referral relationships with all medical staff.

      • Assures the department/clinic meets professional and regulatory standards.

      • Provides timely response to physician and patient correspondence.

      • Completes education necessary to maintain licensure or certification required for position.

      Compensation (Commensurate with experience):

      $107,120.00 - $184,995.20
