      We are enhancing our Urgent Community Response Team and have opportunities within the team for a senior clinician. The post holder will be a senior member of the team. The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care.

      The successful candidate will be an experienced Practitioner with sound clinical knowledge and proven experience in acute and community clinical environments. You will need to be adaptable to operate in a fast-moving environment providing unscheduled holistic clinical care to patients presenting with acute undifferentiated and undiagnosed conditions, complex health needs and /or long-term conditions.

      Main duties of the job

      As senior practitioner you will act as role model supporting the Urgent Community Response team to achieve and maintain high standards of clinical care, based on evidence-based practice.

      As Senior Urgent Community Response practitioner you will support the clinical lead to respond to clinical incidents, accidents and complaints in accordance with CLCH policy.

      Work in collaboration with and maintain a good working relationship within the other Community Adult Health Care Teams within the borough of Harrow its adjoining boroughs, and with the District nursing team, Out of Hours Services, Accident and Emergency Departments, London Ambulance Service, Acute Trust, Community Matrons, General Practitioners and Practice staff, Nursing Homes, Social Services and Voluntary Sector so as to provide a comprehensive service to patients, clients and carers.

      Undertake assessment and appraisal of designated members of the team.

      • Health care professional with current registration ( Nurse ) or similar
      • BSc / Pg Dip Nurse Practitioner Primary Health Care/ Minor injuries / Minor Illnesses
      • Experience in Community and/ or Accident and Emergency

      Desirable

      • Evidence of continuing professional education and training at master's Level.
      • Further desirable qualifications in Primary Care/ Accident & Emergency/Advanced clinical practitioner
      • Qualification in teaching or practice-based learning.

      Experience

      Essential

      • Extensive experience in Community, Primary Care and/ or Accident & Emergency.
      • Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions.
      • Experience of assessing and providing quality care to people with Long Term Conditions
      • Track record of practice excellence.
      • Experience of effectively working in collaboration, negotiating and liaising across organisational boundaries.
