      POSITION SUMMARY STATEMENT:

      Utilizes a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation, and evaluation in accordance with the Nurse Practice Act, scope of practice, along with recognized professional standards and Health Partner and employer specific protocols and policy.

      Supports multiple areas within a clinic setting with strong interpersonal skill and flexibility while maintaining patient confidentiality and protecting patient rights. Resolves concerns or complaints regarding patient care or service.

      Assessment/Nursing Diagnosis

      • Effectively assesses care needs of patients and care givers, prioritizes patient needs utilizing the nursing process; determines nursing diagnosis focused on care and identifies patient outcomes; develops patient care plan; implements and continuously evaluates the care plan; ensures patients are discharged with resolved educational needs
      • Collects and prioritizes data in a systematic and ongoing process that involves the patient, care giver, other health care providers and environment as appropriate
      • Uses appropriate evidence-based assessment techniques and instruments in data collection
      • Analyzes and interprets assessment data to determine nursing diagnoses
      • Formulates and revises nursing diagnoses that reflect the current patient status and communicates this with the patient, care giver and other colleagues within the health care team
        • Develops an individualized education plan for patients and care givers based on identified learning needs

      Planning of Nursing Process

      • Develops plan of care using an outcomes-based approach ensuring quality care and age-related growth and development principles
      • Establishes a plan that provides for continuity of care
      • Uses evidence-based research data blended with caring and compassion, to develop the plan of care and interventions

      Implementation/Evaluation of Nursing Practice

      • Coordinates, delivers, and delegates nursing care that is safe, ethical, and culturally competent based on patient assessment/standards of care in a timely manner
      • Utilizes systematic and ongoing assessment data to revise diagnoses, outcomes and the plan of care
      • Involves the patient, caregiver, and health care team members in the evaluation process when appropriate
      • Utilizes evidence-based interventions and treatments specific to the diagnoses as appropriate
      • Initiates and implements plan of care using an outcomes-based approach ensuring quality care and age-related growth and development principles
      • Provides comprehensive, individualized healthcare teaching, using the teach back method, to patients and caregivers
      • Actively participates in creating and implementing improvements at the point of care to achieve clinical quality, patient experience, nursing engagement, efficiency, and effective outcomes
      • Evaluates the patient’s progress towards attainment of the outcomes and collaborates with members of the care team when outcomes are not as expected
      • Evaluates and documents the patient’s/care givers’ understanding of and response to the plan of care

      Documentation

      • Documents patient care and education accurately, timely, and completely in the EHR (electronic health record)
      • Reassesses, revises, and documents the plan of care based on the patient’s response to treatment and nursing interventions on an ongoing basis

      Collaboration

      • Coordinates and communicates effectively with patient/care giver, clinicians, and the interdisciplinary team in a timely fashion
      • Delegates tasks consistent with the state nurse practice act based on the knowledge, skills and abilities of the designated caregiver
      • Directs, supervises, and evaluates the work of unlicensed assistive personnel in providing delegated nursing services
      • Collaborates/coordinates with other healthcare team members, patient and care givers to determine necessary resources (i.e. social services, home care, type of equipment, information) to support continuity of care

      Professional Practice/Education

      • Practices within the scope of the Minnesota Nurse Practice Act, scope of practice, Health Partners and employer specific policy
      • Attains and applies competency (knowledge, skills, behaviors, and judgement) that reflects current nursing practice
      • Provides tele health management to patients with health concerns or questions. Utilizes the nursing process and approved decision-making tools to assess health care needs based on patient symptoms and history.
      • Participates in educational and professional development activities and maintains required competencies
      • Demonstrates professional and personal development related to empathy, accountability, autonomy, creativity, critical thinking, joy and pride, service and caring, learning, and stewardship
      • Engages in self-evaluation of practice on a regular basis, identifying strengths and goals for professional development
      • Participates and obtains formal and informal feedback regarding one’s own practice from patients, peers, professional colleagues, and leaders
      • Provides immediate and ongoing positive and constructive feedback to other team members regarding their performance
      • Contributes to a supportive and healthy work environment
      • Actively participates in team meetings, trainings, practice development, organizational and unit-based communication
      • Utilizes quality improvement data, evidence-based practice and research to guide decisions and to improve nursing practice and patient outcomes
      Performs other tasks as required or assigned

      MINIMUM QUALIFICATIONS:

      Education, Experience or Equivalent Combination:
      • Graduated from an accredited school of nursing
      • Associate degree or diploma required
      • 1 year of prior experience in nursing preferred
      • Additional qualifications may be applicable for specialty care
      Licensure/ Registration/ Certification:
      • Maintain a current Minnesota registered nurse’s licensure.
      • Current BLS provider status in accordance with American Heart Association (AHA) guidelines
      • No online only courses accepted.
      Knowledge, Skills, and Abilities:
      • Demonstrates knowledge of age-related growth and development necessary to provide appropriate care to the patient population served and ensure safety of patients.
      • Possesses the qualities of strong critical thinking skills and ability to multi-task
      • Ability to communicate effectively both orally and in writing, and maintain standards of clinical skills and professional ethics as established by the Nurse Practice Act
      • Critical thinking, Excellent Interpersonal Skills, Fluent in EMR (EPIC) use
      • Understanding of Nursing Practice Act and Scope of Practice for various roles

      Education, Experience or Equivalent Combination:
      • Bachelor’s Degree in Nursing preferred
      • Ambulatory Nursing Experience preferred
