Job Description

- Provide initial and ongoing assessment of patients, including evaluation of drug therapies, medical conditions, diet, and compliance.
- Provide monitoring of appropriate labs for anticoagulation patients.
- Integrate all appropriate current and past information and apply to established protocols for dosing and monitoring plans.
- Communicate effectively with patient, other care facilities, home care, clinic personnel, pharmacies, and providers regarding care.
- Triage telephone calls from patients, caregivers, or providers
- Provide medication related education and support for anticoagulation patients.
- Provide education and learning opportunities to students on rotation.
- Demonstrate ability to deliver care or services adjusting approaches to reflect developmental level and cultural differences of population served.
- Graduate of an approved school of professional nursing
- One-year RN direct patient care experience
- Current RN license in the state of Minnesota
- Wisconsin RN licensure may be required based on clinic assignment/patient coverage
- BSN preferred
- Previous clinic experience with anticoagulation medications
- Telephone clinic triage experience
- Anticoagulation management certification
OverviewWe at Fairview Health Services are looking for an Anticoagulation Nurse to join our team! This position is a remote/mobile position and the work would be telephonic with the exception of coming onsite one day per month to one of our clinics. This position is approved for 48 hours every two weeks and is benefit eligible! ResponsibilitiesProvide ongoing monitoring and dosage management of anticoagulation patients and communicate drug therapy plans clearly to the patient or caregiver. Provide vital education and support to patients and caregivers while performing ongoing therapy management. Monitor patient's progress toward established goals to achieve optimal outcomes. Maintain complete, timely and accurate clinical documentation for each patient.
