- Coordinate resident admissions & discharges to & from the Arbor by communicating with local hospitals and Galloway Ridge IDT team.
- Welcomes new admissions, orients residents to the Arbor completes required admission paperwork, schedules initial care plan meeting, and enters required assessments into PCC within 48 hours of admission in concert with the MDS coordinator.
- Complete required Medicare paperwork, and assist residents as needed with ambulance appeals, advanced directives, insurance forms, hospice referrals, and talk therapy referrals.
- Complete assigned sections of MDS in a timely manner.
- Act as an integral member of an interdisciplinary team and serve as a resident advocate and liaison. Attends meetings and collaborates with MDS Coordinator or designee to ensure care plan is up to date and reflects resident’s status.
- Keeps Manager informed and up to date, collaborates with IL Navigator to transition residents from IL to the Arbor. Ensures that rooms and apartments are move-in ready, follows the arbor move process guidelines, and provides emotional support and adjustment counseling to new residents and their families as needed.
- Maintain grievance log book in concert with the Administrator to ensure all grievances are addressed appropriately. Informs the Navigation Manager of any new grievances.
- Schedule and facilitate family/resident care plan meetings with the MDS Coordinator on a quarterly basis, bi-annually, or as-needed basis, for residents & enter progress notes into PCC.

- Attends huddles, weekly navigation and case management meetings, and other meetings as assigned
- Perform other related duties as assigned by supervisor or authorized personnel.
- All other duties assigned include, but not limited to, enhancing the Resident Experience.
- Minimum of a Bachelor’s degree in the Human Services field (i.e. Social Work, Psychology, and Sociology) from an accredited school.
- A strong desire to enrich resident lives by providing resident-centered care.
- Excellent organizational, communication, and leadership skills.
- Ability to work independently and as part of a team.
- Experience working with older adults.
- Experience working in a health setting is preferred.

Join our team! Galloway Ridge is an upscale continuing care retirement community located between Chapel Hill and Pittsboro in Chatham County, NC. The Arbor Community Navigator (Social Worker) provides psychosocial support to residents and families adjusting to the Arbor, is responsible for managing changes in health status, and serves as a liaison between the residents and community agencies. Responsibilities include communicating with local hospitals for discharge planning, collecting information to formulate resident social history, and psychosocial assessment, formulation of the resident care plan, completing assigned sections of the MDS, participating in interdisciplinary team meetings, maintaining up-to-date progress notes, communicating with internal and external health care providers, crisis intervention, assist in transitions of care, and any other duties as assigned by supervisor.
