What to Expect (Job Responsibilities):
- Serve as the primary contact for ALTCS members, explaining the program and providing necessary information
- Conduct comprehensive assessments of members' bio psychosocial functioning and document findings
- Develop and implement service plans based on members' strengths, needs, and preferences
- Assist members in defining personal goals and overcoming barriers to achieve them
- Act as a facilitator and advocate for members in dealing with providers and community programs

What is Required (Qualifications):
- Bachelor's degree in social work
- 2 years of experience serving elderly individuals, persons with physical disabilities, or individuals with Serious Mental Illness (SMI)
- Strong communication and interpersonal skills
- Ability to work within budgetary allowances

How to Stand Out (Preferred Qualifications):
- Bilingual language skills
- Additional related education and/or experience
