The Assertive Community Treatment (ACT) Team Supervisor is responsible for overseeing day-to-day operations, oversight, and supervision of the assigned ACT team, which is a community-based mental health team that provides services for individuals experiencing serious mental illness. The mission of ACT is to help people become independent and integrate into the community as they experience recovery through a team-based service model.

      Compensation Range:

      The starting pay for this position is $53,793.95 - $63,287.00 for limited licensed candidates, and $57,321.88 - 67,437.50 annual salary for fully licensed candidates, and is based on non-discriminatory factors such as skills and experience.

      Essential Duties

      • Serve as a leader and role model for the organization in matters relating to ACT services and provide daily supervision of assigned team operations.
      • Ensure the compliance and successful execution of all ACT requirements as outlined in provider contracts and the Michigan Department of Health and Human Services (MDHHS) Medical Manual, including, but not limited to:
        1. Conduct team meetings Monday through Friday on business days and ensure attendance of all ACT staff members on duty, with physicians, nurse practitioners, and/or clinical nurse specialists attending at least weekly, to discuss the status of all beneficiaries, updates from on-call, clinical and case/care management needs, crisis management, schedule organization, and finalized plans for ACT staff deployment into the community.
        2. Ensure that a minimum of 80% of ACT contacts provided by the team are in the beneficiary’s home or other agreed upon community location.
        3. Oversee assigned staff work assignments and work schedules to meet the needs of individuals served and the agency.
      • Assist assigned team members as needed in all aspects of service planning, treatment, and support activities.
      • Provide direct services to beneficiaries in the community within their scope of practice.
      • Ensure that direct reports meet direct service productivity and documentation standards per agency and program expectations.
      • Complete all assigned reporting and projects within designated time frames.
      • Ensure documentation of attendance at required trainings is maintained for all assigned staff and available for review upon request.
      • Regularly assess the quality of work and operational metrics for assigned staff and make suggestions for continuous improvement including the development of team improvement goals.
      • Collaborate with other departments and agencies regarding resources and services for improved patient care and staff development.
      • Attend and conduct meetings in-person in the office or the community, virtually through video (audiovisual), and by phone (voice only) as required.
      • Travel off-site as needed for various programming opportunities including meetings, conferences, activities, and events
      • Ensure assigned staff compliance with quality care initiatives including Meaningful Use (MU), Medicare Merit Based Incentive Payment System (MIPS), Commission on Accreditation of Rehabilitation Facilities (CARF), and Certified Community Behavioral Health Clinic (CCBHC) requirements.
      • Serve as a subject-matter expert on the use of electronic health record (EHR) systems and health registries and promote the utilization of the Community Electronic Health Record (CEHR) for individuals served.
      • Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served, guests, and staff.
      • Participate in and promote departmental and agency quality improvement initiatives.
      • Assume responsibility for professional growth by current reading and attendance at educational programs and conferences.
      • Maintain knowledge and compliance with established policies and procedures, corporate compliance program, code of ethics, applicable federal, state, and local laws and regulations, HIPAA standards, and other regulatory programs.
      • Perform other related duties as assigned.

      Supervisory Duties

      • Provide consistent individual supervision (recorded) at least bi-monthly based on their individual needs and experience, that includes, but is not limited to: a) Monthly case reviews b) Quarterly joint contracts c) Monthly review of progress toward attaining goals d) Review of any corrective action steps.
      • Organize and oversee the day-to-day operations of the assigned team to ensure appropriate coverage of responsibilities.
      • Educate, enforce, and ensure team compliance with CNS Healthcare policies and procedures.
      • Cultivate, maintain, and enhance team, department, and interdepartmental relationships to promote collaboration, teamwork, and a positive working environment.
      • Serve as a conduit of communication between direct reports, department leadership, and agency leadership.
      • Build a strong team by attracting, selecting, onboarding, and developing direct reports to retain high-performance team members and empower them to elevate their level of responsibility, span of control, and performance.
      • Serve as a role model, mentor, and coach in providing ongoing and consistent individual supervision that is focused on meeting individual productivity and performance goals, as well as departmental goals, objectives, and project deadlines.
      • Manage employee performance, including any verbal/written warnings and conducting employee evaluations to provide examples of behavior, ways for employees to improve performance, and development of goals for the coming year.
      • Lead assigned team in quality improvement initiatives and compliance with all quality improvement initiatives and standards to promote quality achievement across the organization.
      • Responsible for managing work schedules, time and attendance, paid time off, and mileage for assigned staff in a timely manner.

      Qualifications:

      Required

      • A Qualified Mental Health Professional (QMHP) with:
        1. Master’s Degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy, AND
        2. State licensure: Limited-Licensed Master’s Social Worker (LLMSW), Licensed Master’s Social Worker (LMSW), Limited-Licensed Professional Counselor (LLPC), Licensed Professional Counselor (LPC), Limited-Licensed Marriage and Family Therapist (LLMFT), Licensed Marriage and Family Therapist (LMFT), OR Limited-Licensed Psychologist (LLP)
      • At least 2 years of post-degree clinical experience with adults who have serious mental illness.
      • Working knowledge of conflict resolution and crisis intervention
      • Must participate in MDHHS-approved, ACT-specific initial training within 6 months of hire, and subsequently participate in at least one MDHHS-approved, ACT-specific training annually thereafter.
      • Experience with the application of the Health Insurance Portability and Accountability Act (HIPAA) in health care organizations
      • Ability to understand and value cultural and ethic differences, alternative perspectives, lifestyles, etc.
      • Proficient with computers and Microsoft office products
      • Proficient in the use of electronic health records (EHRs)
      • Ability to work closely with persons with mental or physical limitations
      • Ability to understand and value cultural and ethnic differences, their alternatives perspectives, lifestyles, etc.
      • Ability to write clear, concise reports and the ability to formulate and implement an effective treatment plan.
      • Ability to work collaboratively and build positive working relationships
      • Ability to recognize the importance of collecting and reporting on outcome data
      • Ability to apply methods for measuring the multiple variables of treatment outcome
      • Ability to communicate effectively, professionally, and courteously
      • Ability to use discretion and judgmental when handling matters of a sensitive or confidential nature
      • Advanced time management, problem solving, customer service, interpersonal, and conflict resolution skills
      • Ability to be forward thinking and take initiative to accomplish goals and objectives of the department
      • Valid and unrestricted Driver’s License
      • Transportation or Automobile accessibility with current automobile/vehicle insurance meeting agency policy requirements

      Preferred

      • Training and/or experience in substance use treatment, stage of change, and motivational interviewing strategies.
      • At least one (1) year of experience in a supervisory role.
