      Job summary

      We are offering an exciting opportunity for an Assistant Care Act Assessor to join our South West Unplanned Care Team. You will work closely with Multi-disciplinary teams/agencies to support those being discharged from hospital to live as independent a life through the promotion of independence; preventing unnecessary hospital admissions and improving the health and wellbeing of the population served.

      As part of the role, you will hold, manage and prioritise a caseload of low complexity/low risk cases; regularly review with adults/carers their individual strengths and needs and maintain records relating to their holistic Care and Support Plan.

      You will be required to work across the geographical areas that we cover and will take responsibility for completing initial reviews of long-term care packages that are in place following the person's time on the D2A pathway.

      Social Work Learning Academy (SWLA)

      On appointment, you will be enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA 'stage' (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. You will be required to take responsibility for your own Continuous Professional Development and will be supported by your line manager to undertake any identified learning.

      Main duties of the job

      As part of this role, you will be skilled to:

      • Compassionately provide social care input and support to adults/carers/families helping to promote self-determination, choice and autonomy.
      • Support individuals to regain skills and become more self-sufficient whilst reducing the dependency on carers and the need for ongoing care at home.
      • Implement, co-ordinate and evaluate patient care and support packages.
      • Review support plans in line with cost effective approaches in line with high quality responsive services to adults within resource availability.
      • Demonstrating initiative and creativity in finding solutions to problems.
      • Work in partnership with agencies external to the Trust supporting adults/carers to access services/support with independent, non-statutory and voluntary sector providers.
      • Liaise with families/carers any other professionals and external organisations ensuring good effective communication and adult involvement takes place.
