- Coordinate and oversee each youth’s intake process, assessment, individual service plan, family reunification, and discharge.
- Train, supervise, and oversee operations of Case Planners and Parent Advocates
- Support Case Planners in continuously identifying and engaging kinship resources.
- Support Parent Advocates in educating birth parents on their required expectations and the services they can access.
- Support Case Planners and Parent Advocates through weekly individual and group supervision.
- Ensure that Case Planning operations, including case contacts, P2P meetings, and Family Time, meet and comply with agency, ACS, and OCFS mandates
- Assess insight into case status, progress, and whether or not a case can progress to the next step toward permanency, such as increased visitations.
- Supports various managerial responsibilities such as but not limited to: ensuring proper case file documentation standards, maintaining appropriate staffing levels, carrying a small caseload as assigned/needed
- Work collaboratively with the treatment team to help families achieve the goal of permanency and reunification
- Develop and maintain relationships with relevant providers and stakeholders, Cayuga Centers staff, and other community programs and organizations that can support families
- Oversee legal case status and mandates of all assigned youth, including those attending court
- Provide supervision and accompany parents when shopping to purchase furniture for new apartments once children are trial and/or finally discharged from care
- Participate in file auditing with the Cayuga Centers Continuous Quality Improvement department (CQI)
- Ensure all contacts and services delivered are documented in relevant electronic health record systems (NETSMART and Connections)
- Support completion of reports, referrals, and other needed documentation for each youth
- Respond to incidents during or outside work hours, and participate in on-call rotations
- Perform other related duties as requested by the Director of Case Planning and senior leadership

About the Program and Position:
The ACS Program supports foster youth in the custody of the Administration for Children’s Services in New York City. Our Enhanced Family Foster Care (EFFC) program serves 137 youth ages 0-21+ and supports them with a multidisciplinary team that includes a Case Planner, Health Services Team, Psychiatrist, Behavioral Support Specialist, Family Advocate, Case Aide, Family Team Conferencing Coordinator, Education Workforce Specialist, Employment/Internship and Housing Specialist, Fair Futures Coach, Foster Home Recruiter and Licensor and Clinician. Our therapeutic approach to foster care includes direct access to Clinicians and foster parents that are trained at a treatment foster care level implementing the ARC framework. This makes a difference because our team approach provides a space for healing while providing skill-building and future-focused planning for youth in our care.
Cayuga Centers currently seeks an Assistant Director of Case Planning who is passionate about supporting foster youth by developing and managing their permanency plans. The Assistant Director of Case Planning will assist in supervising Case Planners and Parent Advocates working within the ACS department and audit case planning procedures to ensure consistent and high-quality services for all youth.
