      This recruitment is to fill one new full-time Assistant Homeless Outreach Coordinator position in the Community Services, Parks & Recreation Department. This person will report directly to the Homeless Outreach Coordinator. Most of the work is performed with clients one on one, along with the Chino Police Department's Quality-of-life team. Must have the availability to work a varied schedule, with days and hours adjusted to meet the needs of the Department. This position will assist with the implementation of program goals and objectives.

      Special Requirements: This position is assigned to work out of the Chino Police Department. 1) Due to the work assignment location, qualified finalist will be required to undergo an extensive police background investigation, which includes a polygraph test and an administrative review. In compliance with the EEOC, a criminal record does not automatically disqualify a candidate from employment. 2) Receive satisfactory results from a physical examination which includes a drug screening. 3) Possess a valid California Driver's License and a satisfactory driving record. 4) Bilingual (Spanish) desirable.


      Definition:

      Under general supervision, coordinates and performs daily outreach to provide homeless services for those affected within the community. Most of the work, from initial outreach and coordinating case management service is performed with clients on street. Maintains effective working relationships with homeless individuals to provide intervention, referral, and case management services. Works under the supervision of the Homeless Outreach Coordinator and alongside Chino Police Department’s Quality-of-Life Team, utilizing available community and county resources to address specific needs.

      The Assistant Homeless Outreach Coordinator must be competent to perform a wide variety of duties to assist the City with homeless services, specifically to reduce the homeless population through available resources.


      Essential Functions:

      • Conduct intakes and assess client needs.
      • Coordinate, monitor, evaluate, and advocate for multiple services to meet the client’s specific need(s).
      • Provide extensive outreach services out in the field.
      • Establish good rapport with diverse homeless population.
      • Effectively plan, provide, and evaluate case management services related to the client.
      • Assist in the implementation of program goals and objectives and provide recommendations for improvement.
      • Transport homeless individuals in City of Chino vehicle to appointments, shelters, rehabilitation centers, etc.
      • Meet with service providers and agencies to facilitate needed services, resources, and opportunities for the client; and establish and maintain effective relationships with governmental and community agencies regarding programs and services.
      • Maintain detailed client service records and prepare reports and correspondence as needed.
      • Ensure the clients’ right to privacy and appropriate confidentiality.
      • Make off-site visits as needed; and assist with facilitating workshops, trainings, and meetings.
      • Participates in public outreach and education.
      • Interact with co-workers at all levels in the organization in a collaborative and customer service-oriented manner.
      • Maintain prompt and regular attendance.
      • Work a flexible, evolving schedule based on the demands of the position including evenings and early mornings.
      • Perform all duties within the context of the City’s Mission Statement and Organizational Values.
      • Perform other related duties as assigned.

      Any combination of education and experience that provides the required knowledge, skills, and abilities to perform the essential job duties of the position is qualifying. A typical example includes:


      Education:

      • High School Diploma or G.E.D.
      • Bachelor’s degree in social sciences, social work, social/behavioral sciences, human services, public administration, or a related field.

      Experience:

      • Case management experience and/or related field expertise.
      • One (1) year of applicable work experience providing case management, social services and working with the homeless in outreach and engagement services (desirable).


      Knowledge of:

      • Case management principles, methods, and techniques.
      • Available medical, housing, psychological, educational, and social service resources.
      • Trends, functions and organization of public assistance and social service systems.
      • Word processing, spreadsheet, and data base computer applications.
      • Pertinent laws and regulations.
      • Narrative case report writing practices.
      • Ethnic and economic groups and cultural differences.


      Ability to:

      • Develop and maintain an effective caseworker relationship with clients.
      • Prepare and implement case management plans.
      • Make decisions and use independent judgment.
      • Establish and maintain cooperative working relationships with community agencies and service providers.
      • Interpret and explain complex laws and regulations to clients and the public.
      • Work effectively in the absence of supervision.
      • Use diagnostic assessment tools to assess physical and psycho-social needs of homeless clients.
      • Communicate effectively, both orally and in writing.
      • Deal tactfully and courteously with internal and external customers.
      • Understand and follow verbal and written directions.
      • Operate a computer using a variety of software programs.
      • Use good judgment in handling highly sensitive and/or confidential matters/records with discretion and maintain the privacy of such information.


      Training/Safety Requirements:

      • Job specific training will be provided according to class specifications during work hours.
      • Possess a valid California Driver's License and a satisfactory driving record.
      • Bilingual (Spanish) desirable.
      • Receive satisfactory results from a background investigation, polygraph test, physical examination which includes a drug screening, and an administrative review, which meet the established qualification standards.

      Physical Profile

      Category II – Moderate Physical Effort: Work assignments for this category require moderate physical exertion associated with the ability to lift, carry, push, pull, or climb.

      Characteristics: Classes in this category require a physical capability for sustained physical work of a moderate nature. Physical requirements may vary from position to position, but do not require the strength and endurance usually associated with heavy physical effort. Moderate physical effort is required while performing tasks such as moving client’s luggage, walking on uneven surfaces, and standing for long periods of time, etc.

      • Lifting and/or carrying of objects weighing up to 25 lbs.
      • Reaching: Reaching above the shoulders to place and/or retrieve objects.
      • Walking: Ability to walk for prolonged periods of time (usually a minimum of two or more hours per day).
      • Standing: Ability to stand with little movement for long periods of time per day (usually a minimum of two or more hours per day).
      • Sitting: Ability to sit with little movement for long periods of time (usually a minimum of two or more hours per day).
      • Work Environment – Outdoors: Ability to work outdoors in all types of weather conditions.
      • Work Environment – Moving Objects: Ability to work around moving objects, machinery, or vehicles.
      • Work Environment – Surfaces: Ability to walk on slippery or uneven surfaces.
