      Job summary

      • To work as part of the Multi-disciplinary Adult Health and Social Care Team to prevent admission and support early discharge from Acute and Community Hospitals.
      • To assist and promote independent social and living skills based on a person centred approach.
      • To assist in the delivery of Nursing and or Therapy Interventions as appropriate.
      • To feedback any outcomes/concerns/progression/deterioration with regard to the achievement of the set goals.
      • To maintain patient records using appropriate documentation for intervention carried out.
      • To enable and empower patients to develop self and environmental management skills.
      • To promote and facilitate access to Community resources.
      • Enable individuals to sustain and improve their overall health and wellbeing.
      • Identify, act and minimise risk to patients and clients.

      We have the exciting opportunity to appoint an Assistant Practitioner, Community Therapy to join our enthusiastic Community Therapy team based in Holsworthy.

      We are looking for a practical, proactive and enthusiastic individual with good communication skills. We are looking for an applicant with experience of working in a Care or Rehabilitation setting with the passion to work in the Community Therapy team based at Holsworthy Community Hospital.

      You will be working with people with a wide range of disabilities and acute or long-term conditions. The role of the Assistant Practitioner is to complete holistic assessments with patients in their home environments. The role holder develops, delivers, monitors and evaluates the outcome of a therapy plan based on the assessment. The role holder manages their own caseload and may delegate to and support B3 clinical support workers. The role holder may deliver 1:1 support.

      The role addresses health promotion, rehabilitation, equipment prescription and urgent, unplanned work to prevent Hospital admission and, or, support discharge. The role includes work in patients homes, residential and nursing homes and remote/virtual working. The post holder will use electronic healthcare systems EPIC as well as paper based documentation. They will be an integral part of the team in terms of service delivery as well as service monitoring and development.

      The Community Therapy team has twelve members including Occupational Therapists, Physiotherapists, an Assistant Practitioner and Clinical Support Workers. We are a supportive team and the role holder will have personal and professional development opportunities. Our integrated Health and Social Care team includes the Community Therapy team, Community Nursing, Community Matron, Rehabilitation Nurse and Adult Social Care team. We work closely with social prescribers in the GP practices for links with and inclusion of the people we work with into community activities. Typically, the day starts with members of the team triaging new referrals and a team meeting to prioritise and plan the day. We positively encourage joint working between members of the community therapy and broader health and social care team.

      We are happy to consider applicants who would be a Band 3 Clinical Support Worker with the expectation the post holder will complete Foundation Degree at which point they will be uplifted to Band 4.

      Working Pattern: 37.5 hours per week, Monday to Friday, 8:00 - 16:00
