- Collects, enters, and reviews IHOP client-level data from different County client record systems.
- Identifies, synthesizes, and records pertinent client information compiled from various County record systems into the IHOP database(s).
- Assesses client eligibility based on administrative data and client records.
- Searches databases for client information requested by case managers to connect clients to housing and services.
- Conducts and analyzes client surveys to evaluate program impact.
- Assists in monthly data auditing, including correcting documentation errors in CHAMP, ORCHID, Care Manager, Microsoft Excel or other systems.
- Organizes and runs CHAMP, ORCHID, Care Manager, or other system reports.
- Assists with preparation of reports and presentations, as needed.
- Assists in researching, compiling, and developing data-based analysis, reports, and the creation and maintenance of data tools.
- Rigorously documents processes and procedures used for all reporting and analyses.
- Other duties as assigned by the IHOP Data and Evaluation Team
- Explores and utilizes diverse data sources for comprehensive reporting, ensuring a holistic representation of program outcomes.
- Gathers and extracts data from various sources, including databases like CHAMP, ORCHID, Care Manager, and Microsoft Excel.
- Collaborates with cross-functional teams to understand data requirements and ensure comprehensive data collection.
- Cleanses and preprocesses raw data to identify and rectify errors, inconsistencies, and missing information.
- Implements data transformation processes to standardize and format data for analysis and reporting purposes.
- Conducts thorough data quality assessments to identify and resolve anomalies, discrepancies, and outliers and develops and implements data validation checks to maintain high-quality data standards.
- Collaborates with HFH clinical providers to identify, track, and analyze Key Performance Indicators (KPIs)
- Conducts statistical analyses of health data for HFH clinical teams, highlighting program value and guiding impactful process changes
- Other duties as assigned by the IHOP Data and Evaluation Team
- Develops tailored reports for internal and external stakeholders, presenting key insights and metrics.
- Generates ad-hoc reports providing snapshot views of program status and supports Quality Improvement (QI) projects.
- Acts as a thought-partner for the clinical teams, finding creative and effective ways to reflect the value of HFH programs.
- Helps refine program management practices through the creation of new reporting and analytical tools.
- Facilitates the design, build, and maintenance of analytical support tools to be used by HFH clinical staff members and by other external stakeholders.
- Provides training and technical assistance on data management and tracking to ensure accurate data collection.
- Convenes and facilitates discussions with stakeholders to design and implement tools and reports.
- Provides technical support to case managers and other staff on data collection systems.
- Assists in designing program reports, analyzing performance, and providing resulting analyses to stakeholders.
- Ensure that all data related needs are met for a new fast moving and wide-reaching Interim Housing Outreach Program (IHOP):
- This is a large body of work with potential for 4,000+ client touches per year in 300+ different locations.
- Assist in the creation of key performance indices (KPI) dashboards for the HFH IHOP program.
- Assist in the development and maintenance of data collection, cleaning, synthesis, and analysis processes to populate KPI dashboards.
- Assist with data-driven continuous quality improvement initiatives for the HFH IHOP program.
- Assist with developing data systems to be used across the program.
- Assist with data analysis for reporting and scholarly presentations.
- Other duties as assigned by the IHOP Data and Evaluation Team
- Real world experience with data management and/or program evaluation in homeless services, social services, public health, health services or related field.
- Working knowledge of quantitative and qualitative research in homeless services, social services, public health, health services or related field.
- Working background in implementation sciences in homeless services, social services, public health, health services or related field.
- Able to successfully collaborate and communicate with interdepartmental partners and external partners.
- Successfully adapts to changing situations and environments.
- Excels in identifying problems, analyzing possible solutions, and deciding on the best course of action.
- Experience developing and managing databases; developing queries to analyze data; matching data; and working across multiple data systems.
- Data analysis and statistical software experience such as SQL, SAS, SPSS, or R.
- Experience with EMR systems and service delivery databases such as HMIS systems.
- Successful clearing through the Live Scan process with the County of Los Angeles.

Salary: $3 5.23 - $50.83 Per Hour
This is a 1-year contract position with an opportunity for renew based on funding.
ORGANIZATION BACKGROUND
The Department of Health Services (DHS) Housing for Health (HFH) program is a cohesive team of clinicians, community advocates, program administrators, and other professionals who collaborate with multiple County departments, community-based service providers, cities, and other partners to provide housing and critical services to People Experiencing Homelessness (PEH) and those who have previously experienced homelessness. HFH works to meet the needs of people with complex health and behavioral health conditions.
PROJECT BACKGROUND
In late 2022, in response to the continued homelessness crisis in Los Angeles County, the Los Angeles County Departments of Health Services (DHS), Mental Health (DMH), and Public Health (DPH) came together to form regional, field-based, multidisciplinary teams called the Interim Housing Outreach Program (IHOP) to address gaps in behavioral health and physical health services among functionally disabled People Experiencing Homelessness (PEH) who are living in interim housing (IH) and connect them to stabilizing services (such as in home caregiving, higher level of care, or enhanced care management/community support). The IHOP teams will comprise of both medical and behavioral health staff and will prioritize and expedite social and housing navigation services in all eight (8) Los Angeles County Service Planning Areas (SPAs). The DHS teams will be funded through a 5-year grant from the state-sponsored Housing Homeless Incentive Program (HHIP) and administered through the two largest Medicaid managed care plans (MCPs) in LA County: LA Care and Health Net.
As part of the initiative for IHOP, Housing for Health will sponsor the Activities of Daily Living (ADL) program to increase access and support for PEH with ADL needs in interim housing (IH). These teams will perform comprehensive assessments of high-risk PEH in IH settings, refer them to supportive services, increase their function and safety within their current environments, and assist them with transitioning them to higher levels of care, as appropriate. The IHOP teams will serve clients in shelters, motels, and tiny homes located in all eight Service Panning Areas (SPAs) in Los Angeles County. DHS-sponsored IHOP staff, hired by the DHS Housing for Health (HFH) program, will include advanced practice providers, nurses, emergency medical technicians, and occupational therapists.
The IHOP assistant staff analyst (ASA) will be a critical member of the IHOP data and evaluation team who track the services rendered by the IHOP team and conduct impact analyses for key stakeholders.

JOB QUALIFICATIONS
The ideal candidate has experience working directly with a client-centric relational database and with managing individuals. They are comfortable with a variety of analytical tools and have taken the lead on projects where the best path forward is unclear. They are a seasoned and effective facilitator and mediator. Specific desirable skills and experience include:
Education/Experience
A Bachelor's degree or higher from an accredited college or university in Public Health, Public Administration, Business Administration, Healthcare Administration, Healthcare Management, Biostatistics, Epidemiology, Social Work, Community Development, Criminal Justice or a related field - OR - One (1) year analyzing and making recommendations that address organizational management issues such as planning, program development, project management, budget development, and program evaluation of health and human services for high-risk populations (e.g. homeless, justice-involved, mentally ill, substance use disorder or medically complex).

WORK ENVIRONMENT
Currently hybrid remote between a home office setting, in office work, and the potential for field work. Work is primarily remote with potential for a future transition to more time spent in an indoors temperature-controlled office setting.
