Job Description
Assists in assessment, planning, implementation and documentation of individual and group activities geared towards facilitating patient recovery.
Job Responsibility
Works with direction and guidance of clinical team to assist in screening, assessment and analyses of patient needs in order to provide appropriate interventions.
Works with clinical team to formulate and help implement individualized therapeutic plans of care, brief interventions and referral to treatment.
Plans and implements individual and group activities in line with therapeutic foundation of program.
Provides skills coaching for patients utilizing the therapeutic framework of the program.
Assists in the evaluation of patient progress and works with and under guidance of clinical team to develop and recommend plan-of-care modifications.
Participates in outcomes research to improve clinical care and assists in program development.
Provides documentation and feedback on patient participation in the activities program and participates in case conferences with other staff members.

Bachelor Degree, required.
