      Job summary

      Nursing associates area new role within the nursing team in England, designed to bridge the gap between healthcare assistants and registered nurses.You will have established expereince of providing hands-on, person-centered care in various health care settings, working with patients of all ages.You will be registered and regulated by theNursing and Midwifery Council (NMC)and have your own distinct standards of proficiency .

      This position offers the opportunity to work flexibly and remotely in supporting Personalised Healthcare Commissioning services to deliver Continuing healthcare.

      You will play a key role in supporting the Continuing Healthcare service across the region. Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.

      You will assist and support in the facilitation of multi-disciplinary assessments and patient reviews, in line with standard operating policy and procedures, and ensuring The National Framework for NHS Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.

      Main duties of the job

      As an Associate Practitioner, you will be expected to have completed a Level 5 Continuing Healthcare Associate Practitioner Apprenticeship, working within a dynamic team, delivering an effective service as part of the NHS Continuing Healthcare team. You must have basic knowledge and experience of working within the health sector and being appropriately assessed against the NHS England CHC Competency Framework. You will have IT and record keeping skills. As an experienced health practitioner you will be able to work independently and as part of the team.

      The post holder should possess basic knowledge of the NHS Framework for Continuing Healthcare and NHS Funded Nursing Care; along with local policy and practice related to CHC.
