Job summary
The Renal Clinical Pathway Administrator(CPA )is responsible for providingadministrative support across the Renal Department including General Nephrology,Renal Frailty , Low Clarance ,Renal Transplant , K- Fit, Renal Vascilar Access .The postholder is required to deliver a high standard of customer care to a wide rangeof client groups, including the stakeholders, multi-disciplinary teams, patients, relatives,and their carers.The postholder will support patients through their hospital care journey, providing thepatient with a single named point of contact who will coordinate their care.Work as part of a team within a dedicated speciality/patient group to ensure that alladministrative functions for patients from the time of GP referral to the time ofdischarge are coordinated in line with specific care pathway instructions to facilitate thesmooth running of the patients' journey. Where a patient has multiple specialist needs,assist the patient in coordinating their pathways to reduce multiple appointmentsacross the Trust
Main duties of the job
The successful candidate must have excellent customer service and communication skills, a high degree of empathy, diplomacy and integrity.
The post holder will be required to demonstrate sensitivity, professionalism and awareness of customer service needs.
In addition, should have good organisation skills and administrative experiences are essential. You will be required have IT skills using Microsoft Office and have experience of similar NHS IT system.
As a member of our team, you will have high personal and professional standards and will seek to support those around you to deliver high quality services.
Role and responsibility:
Manage referrals received by the speciality by working closely with clinicians to ensure that patients are placed on the correct clinical pathway.Improve RTT for Renal services.
