- Knowledge and experience of neurological rehabilitation
- NVQ Level 3 or equivalent in Health & Social Care

- Basic IT skills and confidence using digital systems
- A full UK driving licence and access to a vehicle for work
- Trusted Assessor training or equivalent qualifications

Job summary
Are you passionate about helping others regain independence and improve their quality of life? We are looking for a motivated and caring Band 4 Community Neuro Advanced Rehabilitation Assistant to join our new community team in Swale. This is an exciting opportunity to be part of a dynamic multi-disciplinary service delivering person-centred, high-quality rehabilitation in the community. You will work under the guidance of registered therapists to provide hands-on support and rehabilitation interventions for patients in their own homes. Your role will also include assisting with basic assessments, maintaining service equipment, and being a key point of contact for patients and families.
Main duties of the job
You will support patients by delivering individualised, goal-directed rehabilitation programmes prescribed by physiotherapists and occupational therapists. This includes helping with functional activities, mobility exercises, and basic assessments following Trusted Assessor training. You will monitor patient progress, keep accurate records, and liaise with other members of the multidisciplinary team to ensure coordinated care. The role also includes prescribing and maintaining rehabilitation equipment, completing administrative tasks, and travelling to patient homes as part of your community-based duties.
