      Job summary

      Were looking for Band 5 Nurses tojoin our North Kent Rapid Response Servicewith a base at Livingstone Hospital, Dartford, to help us continue to change things for the better. The Rapid responseteam works across7 days a week from 8am - 8pm.

      We are looking for nurses who have experience within a community setting or a ward based environment.

      Main duties of the job

      Within this role, you will:

      • Manage referrals and caseload, for a7 day per week Rapid Response pathway
      • Assist the Team Leader in running a team, deputizing for the Team Leader in their absence.
      • Actively work as a member of the Rapid Response Pathway to provide high quality nursing care to clients within a community setting, preventing unnecessary hospital admissions and supporting hospital discharges.
      • Be responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
      • Implement, monitor and maintain high standards of care delivery at all times.
      • Care for patientsand their families in the way thatyou believe is best.
      • Deliver our high service standards, sharing bestpractice, and actively improving the way we work.
