- Responsible for the holistic assessment, planning, implementation and evaluation of evidence-based nursing care for patients referred to hospital at home service.
- Responsible for delivering, evaluating and supporting training of patient and carers enabling them to be cared for virtually.
- Utilising remote monitoring as part of the clinical care delivery
- Providing high-quality, nursing care for patients including co-ordination of the multidisciplinary team and allocating visits as appropriate.
- Delivering high-quality, clinically effective care, and patient experience, utilising research and in collaboration with colleagues in everyday practice.
- Participate in effective clinical leadership and management of team members through clinical supervision.
- Actively contribute to the development and improvement of services and the quality of care through reflective practice, clinical supervision and audit.
- Act as a role model, mentoring, teaching, assessing and supporting students and learners on placement and participate in the education, development and mentorship of other staff.

- Recognised NMC Teaching and Assessing Qualification or equivalent
- Evidence of post registration Educational continuing development
- Diploma or Degree in nursing or health related subject
- Clinical Leadership Programme
- Demonstrates a high level of clinical expertise
- Good understanding of clinical governance
- Knowledge of NHS plans and national nursing strategy
- Ability to effectively solve problems
- Demonstrates ability to work with new technology and shows digital competence

Job summary
A new opportunity has been created to provide additional Hospital at home services to improve our patient journeys and improve capacity in our inpatient beds.
As the Band 6 you will be assisting in the development and embedding of this service within the Trust, whilst assessing, advising and teaching patients and families.
Main duties of the job
Virtual wards allow patients to receive hospital-level care at home safely and in familiar surroundings, helping speed up their recovery while freeing up hospital beds for patients that need them most. Patients are reviewed daily by the clinical team and the 'ward round' may involve a home visit or take place through video technology.
