      Job summary

      We are looking for committed and experienced qualified staff to join our established team of mental health professionals to deliver excellent care within LLR.

      As a Mental Health Facilitator, you will work within primary care to assess and support the mental and physical health care for people who are on the SMI register. This will involve working in collaboration with GP practices and primary care colleagues, as well as secondary care services to improve outcomes and peoples quality of life in line with the NHS 10 year implementation plan.

      We are committed within LPT to ensure better outcomes for this client group for their mental and physical well-being, as this group are known to have a reduced life span and to be less likely to access physical health support.

      We currently have a part time position working 15hours per week.

      You should be experienced and able to work autonomously when required whist still working closely as a team of professionals. Support and supervision will be provided and we promote a positive collaborative leadership style.

      There are also training opportunities within this role to be training in decider skills, phlebotomy and non medical prescribing.

      Main duties of the job

      To work autonomously, taking responsibility for care, quality and clinical activity for a designated caseload of service users

      Your main responsibilities are to ensure that you maintain your register of patients across the surgeries and ensure they have an annual mental health and physical health review, whilst working with secondary care teams and GP surgeries.

      You will assign tasks and provide clinical supervision to HCA's working within the team who will help you to complete the physical health checks needed .

      To participate in the effective clinical leadership of the team

      To actively contribute to the improvement of service and quality of care provision to meet the needs of service users, service users and their families

      To act as role model and participate in the education, development and mentorship of other staff

      To work collaborate with other clinicians and within with wider health, social care and voluntary sector communities as appropriate and promote the role of the Mental Health Practitioner to all stakeholders

      To foster a culture of networking in an open and transparent approach with internal and external partnerships and between all stakeholders

      Responsible for the assessment, planning, implementation and evaluation of programmes of evidenced based care to a group of service users according to changing healthcare needs and varying levels of complexity
