Job Description

- Triage patients, using multiple systems to support decision making.
- Coordinate daily allocation of service tasks to other staff including administration, to ensure that all tasks are completed and enabling safe and timely care.
- Lead all monitoring and assessment of patient in the @home service, advising on care and treatment and reporting changes in patient condition as required in line with local protocols and guidance.
- Providing acute care to patients (in their homes) with complex and specialist needs in the community, across Bristol, South Gloucestershire and North Somerset.
- Working alongside the multidisciplinary team to develop, implement and evaluate care plans.
- Assessing the health and psychosocial needs of the patient within the hospital and community setting.
- Plan and implement treatment and use a wide range of service to meet their needs.
- Utilising a wide range of clinical skills to support the ongoing management of patients in their home environment e.g. delivering IV therapies, venepuncture, cannulation and wound care.
- Communicating effectively, networking and liaising to ensure high quality responsive, safe, efficient and compassionate care is provided for our patients, carers and families.

- Degree quailification
- Physical assessment and Clinical Reasoning for Practice Course or equivalent
- Excellent communication, negotiation & influencing skills
- Ability to multi-task with competing demands
- Experience of managing patients in the community
- Ability to clinically supervise staff
- Experience of line management
- Ability to work autonomously
- Knowledge of shared decision making and goal setting, and ability to implement this within patient care
- Able to undertake direct clinical care for acutely unwell service users in the community - e.g. venepuncture, delivery of medications, basic wound care, clinical assessment
- Excellent IT skills
- Experience of digital consultations
- Experience of leading and facilitating teaching for professional groups
- Experience of community care and interface with primary & secondary care
- Access to own car and ability to travel across BNSSG

Job summary
NHS@Home is an innovative service, providing care for people who are acutely unwell in their own homes across Bristol, North Somerset, and South Gloucestershire. It offers an alternative to hospital and helps people return home from hospital more quickly, to.
The service uses a mixture of face-to-face visits, phone and video calls, and remote monitoring through digital technology, to help people recover safely in the comfort of their own home.
NHS@Home has pathways, covering a wide range of conditions including frailty, respiratory, heart failure, OPAT, Infection and general surgical recovery. Our team provide a very wide range of acute healthcare treatments within the home, in addition to monitoring health conditions and vital signs for deterioration.
The service is run in partnership between Sirona care & health, North Bristol Trust, University Hospitals Bristol and Weston NHS Foundation Trust and BrisDoc.
