- Be highly skilled in using and reporting on standardised and non-standardised Physiotherapy assessment to diagnose and analyse highly complex patient needs in the context of activities of daily living.
- Devise Physiotherapy treatment plans and goals in collaboration with the patient, relatives, carers and the interdisciplinary team.
- Assess and issue equipment as appropriate to meet the patients needs and ensure that up-to-date written and electronic records, reports and activity data are maintained in accordance with professional and local standards.
- Be able to create, evaluate all care plans implemented and should have a sound knowledge in discharge planning and ability to make further referrals in to the community as required.
- Use advanced skills in analysing, selecting and grading activity to achieve therapeutic goals.
- Driving Licence preferable as this is a Community post.
- Physiotherapy Qualification and experience mandatory.
- HCPC registration mandatory.
