Job summary

As a Band 6 Physiotherapist you will provide a high standard of physiotherapy assessment, treatment and advice to patients/clients, carers and families. Wherever possible avoid hospital admission and reduce in-patient length of stay to ensure that patients receive the appropriate care in the most appropriate setting. Undertake the lead role in the assessment, diagnosis and formulation of physiotherapy treatment plans across all the different service areas within CRT such as Home based services, Discharge to Assess beds, Woodlands Therapy Centre and to undertake specialist physiotherapy assessments for patients presenting with complex needs eg Movement Disorders, MND, Vestibular pathology, and people who are frail and falling.
Main duties of the job
As a physiotherapist in this role, you'll maintain full professional and legal responsibility for your clinical work, using advanced judgment to assess and prioritise referrals while considering each patients home environment and support system to ensure safe and effective care.
You'll provide physiotherapy leadership within a multidisciplinary team, offering expert guidance and working closely with internal and external partners. Through specialist assessments and clinical reasoning, you'll support person-centred care by developing tailored treatment plans and meaningful goals alongside patients.
Beyond clinical duties, you'll mentor and support colleagues, contribute to education and service development, and uphold high standards through involvement in audits and governance. Collaboration across health and social care will be key to helping patients maintain independence, while ensuring ethical practice and informed decision-making.
You may also support team leadership and service quality efforts, keep accurate records, and ensure safeguarding procedures are followed. Managing therapy equipment safely and confidently, and guiding others in its use, will be an important part of your contribution to patient care

Strong interdisciplinary team player with a background in Community Urgent Care, Discharge to Assess, Reablement, and Falls Service rehabilitation. Aware of team dynamics and skilled in supporting constructive conflict resolution.
