- To work autonomously as a senior practitioner in managing the acute and complex health needs of patients with long term conditions and frailty.
- To act as the lead clinician coordinating personalised management plans to reduce the risk of avoidable hospital admissions.
- To deliver clinical assessment, initiate and manage diagnostics, confirm diagnosis, and recommended clinical management plans that Include prescribing if clinically Indicated, according to personal and professional competencies.
- To be practice as a core member of the Integrated Care Teams, including supporting the Single Point Access (SPA)
- Promotes proactive care and self-management using care plans to identify and act on early signs of exacerbation.
- Works towards further specialism in the management of LTCs and service speciality development.
- Demonstrates high level of knowledge & skills within area of speciality / area of practice.
- Demonstrates a high level of safe and effective clinical decision making.
- Underpinning knowledge of the concepts of advanced practice.
- Working towards competency in advanced clinical nursing skills with high level of clinical skills in area of expertise including Advanced Physical Examination Skills.
- History taking and differential diagnosis at Level 7.

Job summary
Are you passionate about delivering high-quality healthcare in the community? Were excited to offer a fantastic opportunity for the role of an experienced Band 7 Community Matronor a developmental role for an experienced District Nurse or Community Matronto join our team within Adult Community Services in Surrey.
As a Community Matron, you'll play a pivotal role in the delivery of care within our Integrated Care Teams (ICT's) across Surrey Heath, working alongside multiple internal and external services.
Your expertise and compassion will make a significant difference in improving the health outcomes and quality of life for individuals under your care.
You will need to be an experienced practitioner, with proven clinical abilities in the management of patients with complex health needs and long-term conditions supporting prevention of avoidable hospital admissions.
This role covers theSurrey Heath locality, and you will be expected to travel across the area as required, therefor having a driving licence and access to a vehicle is essential
