      Job summary

      General Practitioner Intermediate Care Beds - Bank Are you a GP with intermediate care beds experience? We are looking for a GP who will join our bank team and care for our patients in Byng House & Chase Heys . You will be the patients GP for the duration of their stay with the aim of maximising their wellbeing and supporting their rehab and discharge. You will form part of the multidisciplinary team caring for patients, working closely with the local Trust and community teams to support the safe set-up and step down of patients to the bed base.

      Main duties of the job

      Provide high quality holistic clinical care, using skills and experience in accordance with the GMCs Good Medical Practice (2024), demonstrating care, compassion, dignity and respect at all times. Work as part of a multidisciplinary team to facilitate the safe and timely admission, rehabilitation and discharge of patients Provide medical oversight and support for the assessment, treatment and management of patients within the PC24 bed base. Undertake a variety of duties including: Conducting ward rounds both independently and with wider MDT colleagues Completing clinical reviews and summarising patients current and previous treatment Formulating comprehensive management plans with a focus on patient centred care. Referring to different clinical disciplines for expert opinion when required o Prescribing medication, including anticipatory medication, with careful consideration of patient needs. Ordering and interpreting diagnostic tests including radiology and pathology requests and acting on results promptly. Making appropriate referrals to secondary care or other agencies when necessary. Recognising and assessing deteriorating patients and escalating care as needed. Initiating end of life care with compassion and respect for patient dignity. Engaging in safe and effective discharge planning with appropriate handover. Completion of statutory certification promptly and accurately. Maintain accurate and contemporaneous electronic medical records with the PC24 clinical system. Perform any other duties as agreed with the Medical Director, Clinical Lead or Head of Service to meet the needs of the service/organisation What do I need Full GMC Registration and licence to practice MRCGP qualification On the performers list Experience in a similar environment Up to date enhanced DBS certificate Understanding of clinical risk management and clinical governance Ability to take independent clinical decisions when necessary and to seek further advice from colleagues as appropriate Ability to manage own time and workload in line with SLAs Excellent interpersonal skills ability to communicate sensitively Demonstrate skills in written and spoken English adequate to enable effective communication Competent in the use of IT systems including ADASTRA and EMIS systems
