- Possess a Master’s degree from an accredited college or university with a major in Social Work; AND Have an LLMSW or LMSW from the State of Michigan Department of Consumer and Industry Services. LLPC/ LPC also accepted.
- Must meet qualifications of a Child Mental Health Professional (CMHP) and/or Mental Health Professional (MHP) per the Michigan PIHP/CMHSP Provider Qualifications per Medicaid Services
- Applicant must have an Infant Mental Health Certification and possess a minimum Level II Endorsement in Infant Mental Health OR possess a minimum Level II
- Endorsement in Infant Mental Health.
- Possess a valid motor vehicle operator or chauffeur's license, and auto insurance.
Must possess knowledge and have an understanding of co-occurring disorders (mental health and substance abuse). - Provide home- and community-based, relationship-focused therapy to children ages 0–6 and their caregivers
- Deliver individual, family, and group therapy, as well as case management and support services
- Maintain MDCH-required home-based service hours based on the child’s medical necessity
- Conduct clinical assessments, develop diagnoses, and create person/family-centered treatment plans
- Monitor client progress and coordinate behavioral health and related services
- Maintain accurate, up-to-date documentation in both electronic and paper charts
- Participate in individual, reflective supervision and team meetings to strategize client support
- Provide 24/7 crisis intervention support on a rotating basis
- Ensure documentation meets funder and Easterseals MORC standards
- Authorize and deliver only medically necessary and approved therapeutic services
- Use trauma-informed approaches and support clients with co-occurring and trauma-related issues
- Complete MichiCANS/PECFAS assessments quarterly for all clients
- Travel throughout the service area using a personal or agency vehicle to deliver community-based care
